摘  要:目的通过比较结合了超声弹性成像(UE)结果的乳腺影像学报告及数据系统(BI-RADS)诊断标准与BI-RADS对乳腺癌的诊断效率,期望发现适合我国妇女的更好的诊断标准。方法回顾性分析有手术病理结果的629例患者共732个病灶(良性508个,恶性224个)的常规超声检查结果,按照BI-RADS评分标准进行评分;UE检查结果以改良5分法为标准进行评分,结合UE结果对BI-RADS评分进行调整,得到新的评分。构建受试者应用曲线(ROC),比较2种诊断标准的诊断效率。结果 BI-RADS标准ROC曲线下面积(AUC)0.882,结合UE后的改良BI-RADS的AUC 0.978,二者之间的差异有统计学意义(P<0.05)。改良BI RADS对乳腺肿物诊断的特异性、准确性要高于BI-RADS诊断标准(P<0.05)。结论结合UE结果的改良BI BRADS标准可以提高乳腺癌的诊断效率。Objective To find a more suitable standard for Chinese women through comparing the diagnostic value of BI-RADS and the improved BI-RADS combining the results of ultrasonic elastography(UE). Methods 732 lesions(508 benign, 224 malignant) from 624 consecutive patients were analyzed. The lesions were scored with BI- RADS and improved 5-scoring system respectively, then scored with the improved-BI-RADS combining the UE results. The diagnostic performances of the 2 BI-RADS standards were evaluated with receiver operating characteristic curve (ROC). Results The areas under the curve (AUC) of the 2 methods were BI-RADS 0. 882, improved BI-RADS 0. 978. The diagnostic performance of the improved BI-RADS was better than that of BI-RADS (P〈0.05), the specificity and accuracy of the improved BI-RADS were better than those of BI-RADS (P%0.05). Conclusions The improved-BI-RADS combining the results of UE could improve the diagnostic of breast cancers.
