阴道封闭术对盆腔器官脱垂患者排尿困扰症状的疗效观察

摘　　要：目的探讨阴道封闭手术对盆腔器官脱垂(Pelvic Organ Prolapse,POP)患者排尿困扰的疗效。方法 2005年10月-2010年2月间采用盆底功能障碍症状问卷排尿困扰量表(Urinary Distress Inventory,UDI-6)和盆腔器官脱垂困扰量表(Pelvic Organ Prolapse Distress Inventory,POPDI-6)以及生活质量问卷排尿影响问卷(Urinary Impact Questionnaire,UIQ-7)和盆腔器官脱垂影响问卷(Pelvic Organ Prolapse Impact Questionnaire,POPIQ-7),对60例因POP-QⅢ-Ⅳ期重度POP实施阴道封闭手术患者,分别于术前、术后2个月、6个月及1年进行调查。结果术后2个月、6个月及1年时随访率分别为93%(56/60)、88%(53/60)和87%(52/60)。术后2月时,全部患者排尿梗阻症状消失,尿频、尿急及压力性尿失禁明显缓解并维持至术后1年。手术前后的排尿困扰症状及生活质量评分显著下降,差异有统计学意义(P=0.000 0)。结论阴道封闭手术可有效缓解POP患者排尿困扰,显著改善患者生活质量。Objective To study the therapeutic effect of colpocleisis on urinary symtpoms in patients with pelvic organ prolapse （POP）. Methods From October 2005 to February 2010, 60 patients with stages III and IV POP diagnosed with POP-Q system underwent total and partial colpocleisis. Outcomes of the patients were compared using the urinary distress inventory（UDI-6）, pelvic organ prolapse distress inventory（POPDI-6） and urinary impact questionnaire（UIQ-7）, and pelvic organ prolapse impact questionnaire（POPIQ-7） 2 months, 6 months and 1 year respectively before and after operation. Results The patients were followed up for 2 months, 6 months and 1 year respectively after operation, with a follow-up rate of 93%（56/60）, 88%（53/60） and 87%（52/60） respectively. The obstructive urinary symtpoms disappeared completely 2 months after operation. The other symptoms such as frequency and urgency of urination as well as stress incontinence improved significantly 2 months after operation and maintained for 1 year. The mean score for urinary symtpoms and quality of life of the patients was significantly higher after operation than before operation（P=0.000 0）. Conclusion Colpocleisis can effectively improve the urinary symtpoms and quality of life of patients with POP.
