摘  要:目的观察慢性阻塞性肺疾病急性加重期(AECOPD)患者血清E-选择素(E-selectin)和血管细胞黏附因子-1(VCAM-1)水平,以及银杏叶提取物(GBE)对其的影响。方法将60例AECOPD患者随机分为常规组和GBE组,各30例;并选择同期30例健康体检者作为对照组。两组基础治疗相同,GBE组每日加用舒血宁注射液20 ml静脉滴注,连用2周。应用酶联免疫吸附法检测血清E-selectin和VCAM-1的水平。两组均于入院第1天和治疗2周后行肺功能检查。结果治疗前常规组和GBE组E-selectin和VCAM-1水平均较对照组高(均<0.01);治疗后,两组E-selectin和VCAM-1水平均较治疗前显著降低(均<0.05),而GBE组较常规组下降更为显著(均<0.05)。两组1 s用力呼气量预测值、1 s用力呼气量/用力肺活量均较治疗前高,GBE组较治疗前差异有统计学意义(均<0.05),而常规组治疗前后无显著差异。结论银杏叶提取物可明显降低AECOPD患者血清E-selectin和VCAM-1水平,改善肺功能。Objective To investigate the influence of ginkgo biloba extract(GBE) on serum e-selectin and vascular cell adhesion molecules-1(VCAM-1) of patient swith chronic obstructive pulmonary disease(COPD) in acute exacer-bation.Methods Sixty patients with COPD at acute exacerbation stage were randomly divided into two groups: con-ventional treatment(CT) group and GBE treatment(GBE) group,with 30 cases in each group,and 30 healthy adults served as the controls.All COPD patients were given conventional treatment,while for the patients in GBE group,20 ml GBE were given through intravenous drip everyday in addition for 2 weeks.The levels of the serum E-selectin and VCAM-1 in the blood samples were determined with enzyme linked immunosorbent assay(ELISA),and the pul-monary function of both groups were measured before and after the treatment.Results The level of E-selectin and VCAM-1 were significantly higher in COPD patients with acute exacerbation compared to the control group before treatmen(t 0.01).After intervention,the level of E-selectin and VCAM-1 in both treatment groups were signifi-cantly decreased(all 0.05),and the decrease in GBE group was more prominen(t 0.05).After treatment,The level of FEV1% and FEV1/FVC in GBE group increased( 0.05),and those in the CT group were not signifi-cantly different( 0.05).Conclusions GBE may obviously decrease the serum level of E-selectin and VCAM-1 and improve the pulmonary function in patients with COPD at acute exacerbation stage.
