摘 要:目的:观察复方鬼箭羽合剂对早期糖尿病肾病的治疗作用,以及探讨这种治疗作用的机制。方法:单侧肾切除STZ腹腔注射造模的方法,采用肾小球光镜和电镜病理染色的方法、以及Smad4和Smad7免疫组织化学染色的方法观察方法复方鬼箭羽合剂对肾小球Smad4和Smad7表达的调控作用。结果:8、12周末模型组大鼠肾小球内Smad4阳性相对面积显著高于正常对照组(P<0.01),复方鬼箭羽合剂治疗组大鼠肾小球内Smad4的蛋白表达量较模型组显著降低(P<0.01),8、12周末模型组大鼠肾小球内Smad7阳性相对面积显著低于正常对照组(P<0.01),复方鬼箭羽合剂治疗组大鼠肾小球内Smad7阳性相对面积与正常对照组差别并不显著(P>0.05),而12周时复方鬼箭羽合剂治疗组大鼠肾小球内Smad7阳性相对面积较8周有所下降,有显著性差异(P<0.05)。结论:复方鬼箭羽合剂治疗组能显著上调Smad4和使Smad7蛋白的表达降低,说明复方鬼箭羽合剂可通过调节Smad4和Smad7蛋白表达,从而降低TGF-β1的表达,达到防治糖尿病肾病肾小球硬化的作用。Objective To explore the therapeutic effect of herbal Euonymi Ramuli Compound(ERC) on diabetic nephropathy and its mechanism.Methods 64 Sprague-Dawleg(SD) rats were divided into normal control group,Diabetic nephropathy(DN) model group,Euonymi Ramuli Compound treatment group and Benazepril treatment group.DN model rats were prepared by intraperitoneal injection with STZ after unilateral nephrectomy.After one week,DN model rats of treatment groups were treated with Euonymi Ramuli Compound and Benazepril respectively for 12 weeks.The renal morphological changes were observed under microscopy and the ultrastructure was observedunder electron microscope.Immunohistochemical staining was performed to explore the excess expression of components of extracellular matrix in mesangial cells such as Smad4,Smad7 proteins.Immunofluorescence(IMF) was applied to unveil the immunological damage in glomerulus of kidney.HPIAS-2000 image analysis system was used for analysis of the expression of glomerular cytokines.Results Immunohistochemistry results showed that Euonymi ramuli compound and Benazepril can down-regulate the expression of Smad4 and up-regulate the expression of Smad7 in treatment groups,comparing with the model group(P 0.01).Conclusion Euonymi Ramuli Compound may exert its therapeutic effect on DN by means of down-regulation of Smad4 expression and up-regulation of Smad7 expression in DN rat kidney.
