体外受精-胚胎移植后异位妊娠的危险因素分析

摘　　要：目的探讨接受体外受精-胚胎移植技术助孕患者异位妊娠的发生率及影响因素。方法对体外受精-胚胎移植(in vitro fertilization and embryo transfer,IVF-ET)后270例妊娠病例的临床资料进行回顾性分析。结果 270例妊娠病例中,宫内妊娠253例,异位妊娠17例,异位妊娠的发生率为6.29%。有流产史患者的异位妊娠发生率为15.7%,无流产史患者的异位妊娠发生率为3%,两组相比差异有统计学意义(P<0.05);曾接受输卵管整形术(包括介入疏通术及输卵管造口术)患者的异位妊娠发生率为20.8%,无该类手术史患者的异位妊娠发生率为4.9%,两组相比具有统计学差异(P<0.05);输卵管积水患者的异位妊娠发生率为29.4%,显著高于无此因素患者异位妊娠发生率的4.7%(P<0.05)。结论 IVF-ET术后异位妊娠的发生与既往有流产史、输卵管整形史及输卵管积水有关。Objective To investigate the risk factors related to the occurrence of ectopic pregnancy among women who received in vitro fertilization and embryo transfer（IVF-ET）. Methods A retrospective study of 270 IVF-ET pregnancies was performed. Results Of 270 IVF-ET pregnancies,17（6.29%） were found ectopic pregnancies.The rate of EPs was 15.7% in abortion group,and 3% in common group（P〈0.05）.The rate of EPs was 20.8% in oviduct reshaping pregnancies and 4.9% in the other pregnancies（P〈0.05）.The rate of EPs was 29.4% in hydrosalpinx and 4.7% in the others without hydrosalpinx（P〈0.05）. Conclusion Previous abortion history,oviduct reshaping surgery and hydrosalpinx are related to the occurrence of ectopic pregnancy following IVF-ET.
