摘  要:目的探讨门静脉血流阻断对梗阻性黄疸大鼠肝细胞凋亡的影响。方法 Wistar大鼠60只,分为A组:胆道再通、门静脉周围游离、切除肝尾状叶,90 min后关腹腔;B组:胆道再通、肝尾状叶分流门静脉、余肝门静脉血流阻断90 min、恢复正常门静脉血流、切除肝尾状叶,关腹。术后0、1、6、24、72 h采集标本,进行组织学观察,测定肝功能及内毒素水平变化并利用TUNEL法检测肝细胞凋亡。结果术后B组肝功能损害更明显(P<0.05),内毒素水平更高,且肝细胞凋亡也更明显。结论 胆道梗阻条件下门静脉血流阻断可能通过损伤肝细胞线粒体导致肝细胞凋亡。Objective To determine the effect of portal vein occlusion(PVO) on hepatocellular apoptosis in rats with obstructive jaundice(OJ).Methods Sixty rats were randomly divided into two groups,group A and group B.The rats of group A received ligation of common bile duct,and in 1 week after which followed by biliary tract recanalization,liberation of portal vein,resection of caudate lobe of liver and then closure of abdomen in 90 min later.For the animals of group B,they were given portal vein bypass in caudate lobe of liver,PVO of left hepatic tissue in 90 min,then recovery of portal vein flow and resection of caudate lobe of liver.Liver tissue and blood samples were collected at 0,1,6,24 and 72 h post operation,one time point for 6 rats.The histological changes were observed by light and transmission microscopy,hepatic function and serum endotoxin level were measured by biochemical methods,and hepatocellular apoptosis were detected by TUNEL.Results Compared to group A,group B presented more severe damaged hepatic function,higher level of endotoxin and more obvious hepatocellular apoptosis.Conclusion In the condition of OJ,PVO may damage hepatocelluar mitochondria which leads to cell apoptosis.
