脉冲Nd∶YAG激光多点布阵术与下鼻甲部分切除术治疗慢性肥厚性鼻炎的疗效比较

摘　　要：目的比较脉冲Nd∶YAG激光多点布阵术与下鼻甲部分切除术治疗慢性肥厚性鼻炎的疗效。方法慢性肥厚性鼻炎患者206例随机分为两组,脉冲Nd∶YAG激光多点布阵(激光组)99例,下鼻甲部分切除(手术组)107例。结果术后6个月,激光组和手术组总有效率分别为97.9%和97.2%,差异无显著性意义(P>0.05)。激光组和手术组术后主要并发症(出血、鼻腔干燥、粘连)的发生率分别为8.0%和30.8%(P≤0.01)。其中激光组出血率为1.0%,手术组出血率高达15.0%,差异有非常显著意义(P≤0.01);鼻腔干燥和粘连两组的发生率差异无显著意义(P>0.05)。结论脉冲Nd∶YAG激光多点布阵术与下鼻甲部分切除术治疗慢性肥厚性鼻炎的疗效相同,但前者具有术中出血少、术后并发症发生率低、手术时间短、容易操作、术后反应小和恢复快等优点。Objective To evaluate the value of clinical application of pulsed Nd ∶YAG laser multipiont array to treat chronic hypertrophic rhinitis.Methods A total of 206 patients were divided into 2 groups randomly.These patients were treated by pulsed Nd ∶YAG laser multpoint array（laser group,99 cases） and by partial inferior turbinectomy（operation group,107 cases）.Results Six months follow-up after operation,the total effective rate of the laser group was 97.9%,and that of the operation group was 97.2%.Two groups of curative effect didn＇t show signficant difference（P0.05）.The incidence rate of the main complications after operation（hemorrhage,xeromycteria and adhesion）was 8.0% in the laser group.It was obviously lower than 30.8% in the operation group（P≤0.01）.The incidence rate of hemorhape was 1.0% in the laser group and was 15.0% in the operation group.There was very significant difference between two groups（P≤0.01）.The incidence rate of xeromycteria and adhesion didn＇t show significant difference between two groups（P0.05）.Conclusions The curative effect of the pulsed Nd ∶YAG laser multipoint array and partial resections of the bottom turbinate to treat the chronic hypertrophic rhinitis is the same.The laser group has the advantages of less bleeding,low complication rate,short operation time,easy manipulation,mild response and quicker recovery.
