摘  要:目的:分析游离髓核MRI表现以提高诊断准确率。方法:手术证实的40例腰骶段游离髓核病例纳入研究,男性28例,女性12例,年龄26~74岁,平均48岁。采用美国Marconi 1.5T超导型磁共振成像系统和表面线圈,矢状面常规行FSE T1WI及T2WI或STIR,横断面行T2WI扫描;其中10例同时进行增强T1WI扫描。分析游离髓核的游走方向、形态、信号特点,源椎间盘的信号、高度改变等。结果:40例游离髓核大多呈不规则的、拉长的形态,少数呈圆形;游离髓核向上游走13例,向下游走24例,位于源椎间盘后方3例;仅1例位于硬膜囊后方。AES分区:左隔区22例,右隔区14例,左右隔区4例;其中29例对中线隔有压迫,11例对中线隔无压迫,所有病例对后纵韧带均有压迫;4例两侧神经根受到压迫,2例未见明显压迫,14例右侧神经根受压迫,20例左侧神经根受压迫;10例增强扫描显示4例未见明显强化,6例病灶周边呈环形强化。结论:MRI能清晰显示各种游离髓核,为诊断和治疗提供重要信息。Purpose: To analyze MRI findings of the free disc fragments for improving diagnostic accu- racy. Methods: Forty patients with free fragment discs at lumber spine were performed MRI examination with American Marconi 1.5T MR set. Surface coil was used. All imagings were obtained with sagittal FSE T1WI, T2WI and/ or STIR sequence, axial T2WI.Ten of all cases were examined after the injection of Gd - DTPA. MR findings of all cases with free disc fragments were analyzed. Results: Of 40 cases, most cases had an irregular, oblong shape, and the rest were round. Thirteen free fragment discs moved in a superior direction, 24 in a inferior direction, 3 were very close to the donor discs. Twenty - two free disc fragments occurred in the left septum, 14 in the right, 4 in the central. The midline septum was com- pressed in 29 cases, and the PLL was compressed in all cases.Both sides of the nerve roots were com- pressed in 4 cases, the right nerve root in 14 cases and the left nerve root in 20 cases, 2 cases had no compression to the nerve root. Of the ten cases with contrast - enhanced scan, six cases showed the ring enhancement, while four cases had no enhancement. Conclusion: MRI can show the free disc fragments well and orovide imoortant value to the diagnosis and the treatment.
