鼻内窥镜在鼻内翻性乳头状瘤治疗中的价值

摘　　要：目的：探讨鼻内窥镜在鼻内翻性乳头状瘤手术中的价值。方法：参考Krouse分期法对43例鼻腔鼻窦内翻性乳头状瘤患者进行分期,根据分期行单纯鼻内镜手术、鼻内镜联合柯-陆进路手术、鼻侧切开术。术后规范随访6~42个月,比较复发率。结果：临床分期相同的患者3种术式疗效无显著性差异（P〉0.05）。总复发率9.3%（4/43）。结论：鼻内窥镜下手术是治疗Ⅰ~Ⅱ级鼻内翻性乳头状瘤一种较好的方法;对Ⅲ级病例,联合柯-陆进路可取得较好疗效。鼻内镜术可为Ⅳ级NIP提供辅助作用,鼻内窥镜手术治疗复发病例仍有较好效果。Objective： To study the value of nasal endoscopy in the surgery of the nasal inverted papillomas.Methods： Total 43 cases of NIP were analyzed and divided into 4 different phases based on the Krouse criterion.The cases were treated by three kinds of surgical procedures： single endoscopic surgery,endoscopic plus Caldwell-Luc surgery,and lateral rhinotomy.All the patients were followed up for 6-42 months after operation.Results： There was no obvious difference on the therapeutic effect of three kinds of surgical procedures in same stage（P0.05）.The total recurrence rate was 9.3%（4/43）.Conclusion： The endoscopy was a better method for treating Ⅰ-Ⅱ stage nasal inverted papilloma.Better results for patients in stage Ⅲ would be achieved by combining endoscopic technique with Caldwell-Luc surgery.Endoscopy can play a helpful role in Lateral rhinotomy.Endoscopic surgery has achieved good effects for recurrence cases.
