摘 要:目的采用组织芯片技术观察血小板源性生长因子(platelet derived growth factor,PDGF)在普通型间质性肺炎(usual interstitial pneuminia,UIP)和非特异性间质性肺炎(nonspecific interstitial pneumonia,NSIP)患者肺组织中的表达及其意义。方法选择经临床-影像-病理诊断的特发性间质性肺炎47例,其中UIP 25例(UIP组),NSIP22例(NSIP组),制作组织芯片,采用免疫组织化学方法检测PDGF在两组肺病灶组织中不同细胞成分的分布和表达情况,用PGM-1检测肺泡巨噬细胞(alveolar macrophages,AM)在肺组织的分布。结果 PDGF主要分布在支气管上皮细胞、肺泡上皮细胞、间质细胞、AM及部分成纤维细胞、血管内皮细胞和平滑肌细胞,PDGF在NSIP组上述各类细胞表达均强于UIP组,两组比较差异均有统计学意义(P<0.05)。PDGF在两组患者支气管上皮细胞和肺泡上皮细胞上的表达呈正相关性,且在AM上的表达最强。结论 PDGF在UIP和NSIP患者肺纤维化过程中发挥重要作用,AM是其重要来源之一,两组间的差异表达为临床病理采用免疫组织化学及分子病理学等技术方法做诊断及鉴别诊断提供了参考。Objective To investigate the expression and significance of platelet derived growth factor (PDGF) in usual interstitial pneuminia (UIP) and nonspecific interstitial pneumonia (NSIP) by application of tissue microarray technology. Methods Forty-seven patients with idiopathic interstitial pneumonia were enrolled in the study, including 25 cases of UIP and 22 cases of NSIP. The distribution and expression of PDGF in different types of cells of lung tissues were examined by tissue microarray methods using immunohistochemical staining. The distribution of phosphoglucomutase-1 (PGM-1) in alveolar macrophages (AM) of lung tissues was detected by using immunohistochemical staining. Results PDGF were localized in bronchial epithelial cells, alveolar epithelial cells, pulmonary interstitial cells, AM, some fibroblast cells, vascular endothelial cells and smooth muscle cells. Its expression in NSIP was stronger than that in UIP. There was a significant difference in positive expression of PDGF between UIP and NSIP groups ( P 〈 0.05). The expression of PDGF in bronchial epithelial cells was positively correlated with that in alveolar epithelial cells in both groups, and AM showed the strongest positive expression. Conclusion PDGF is expressed in UIP and NSIP, and one of the chief sources of PDGF in lungs was AM. The different in expression intensity between two groups may provide the reference for differential diagnosis.
