微血管减压治疗神经原性高血压临床分析

摘　　要：目的：探讨神经原性高血压的外科手术治疗。方法：对１８ 例神经原性高血压伴有三叉神经痛、面肌痉挛、舌咽神经痛的病人进行了显微血管减压手术治疗。其中１２ 例左侧， ６ 例右侧， 术中均发现异常血管襻压迫神经根及延髓， 用涤纶棉隔离压迫。结果： 在术后相应神经疼痛、抽搐症状缓解情况下， 经１～５ 年随访， １６ 例血压恢复正常， ８ 例减少了降压药服用量， 所有病人无严重并发症。结论： 神经原性高血压病人， 排除其他原因所致的继发性高血压， 对伴有三叉神经痛或面肌痉挛， 或舌咽神经痛等患者， 可考虑微血管减压手术。Objective:To discuss the effect of surgical treatment of neurogenic hypertension.Method:Eighteen cases of neurogenic hypertension were treated with microvascular decompression,among those cases,13 with trigeminal neuralgia,3 with hemifacial spasm and 2 with glossopharyngeal neuralgia. Arterial loops compression of nerve root and (or )medulla was found,and a treatment of microvascular decomtression with Teflon was used in all patients.Result:The followed-up period were 1～5 years.The blood pressure remains normal at present in 16 patients.2 patients on a little medicine.No serious complication in all cases.Conclusion:The microvascular decompression may be a useful way to operate of neurogenic hypertension with trigeminal neuralgia,hemifacial spasm and glossopharyngeal neuralgia.
