单纯超滤治疗难治性心力衰竭的临床观察

摘　　要：目的探讨单纯超滤治疗难治性心力衰竭的疗效与安全性。方法选取20例难治性心力衰竭患者,给予单纯超滤治疗,治疗前后比较患者一般情况、左室射血分数(LVEF)。结果 20例患者共超滤95例次,透析后尿量、体质量、LVEF值较治疗前好转(P<0.05或P<0.01)。发生低血压6例次,恶心呕吐4例次,出现2例次心绞痛和急性肺水肿,经对症治疗后好转。结论单纯超滤治疗难治性心力衰竭是一种有效方法,但需结合具体病情调节,以预防低血压、心绞痛、急性肺水肿等的发生。Objective To investigate the efficacy and safety of pure ultrafiltration in refractory heart failure.Methods20 cases patients with refractory heart failure were given pure ultrafiltration treatment,before and after treatment,compared general situation,left ventricular ejection fraction（LVEF） of patients.Results 20 cases patients were given pure ultrafiltration 95 times,and dialysis,urine,body mass,LVEF values were better than those before treatment（P〈0.05 or P〈0.01）.6 cases of hypotension,nausea and vomiting 4 cases,1 case occurred angina and acute pulmonary edema,the symptomatic improved after treatment.Conclusion Pure ultrafiltration in refractory heart failure is an effective method,but with specific conditions to be adjusted to prevent low blood pressure,angina,acute pulmonary edema.
