46例慢性阻塞性肺疾病合并下呼吸道真菌感染的临床分析

摘　　要：目的探讨慢性阻塞性肺疾病(COPD)并发下呼吸道真菌感染的因素。方法选择46例COPD并发下呼吸道真菌感染患者为试验组,32例单纯COPD患者为对照组,比较抗菌药物、激素使用和呼吸机辅助通气的情况。结果试验组和对照组在抗菌药物使用种类、持续时间、激素使用时间、呼吸机辅助通气方面差异均有统计学意义;试验组治疗后sTREM-1为(8.95±3.77)pg/ml,显著高于对照组治疗后,试验组治疗后降钙素原与C-反应蛋白分别为(1.57±1.04)ng/ml和(50.68±21.41)mg/L,均显著低于治疗前。结论对于治疗效果不佳、抗菌药物或者激素使用时间长、使用呼吸机的COPD患者,应及时进行病原学检查,早期诊断真菌感染并及时治疗。OBJECTIVE To discuss the risk factors of the chronic obstructive pulmonary disease complicated with lower respiratory fungal infection. METHODS A total of 46 cases of chronic obstructive pulmonary disease merger lower respiratory fungus infection were selected and simplicity in patients with chronic obstructive pulmonary disease 32 cases. The antibiotic usage circumstance, hormone usage and ventilator assisted entilation situation were compared. RESULTS Experimental group and control group in antibiotic use kinds, duration, and hormone use time were significant differences. Experimental therapy for sTREM-1 after （8.95±377） pg/ml, and before treatment compared the non-significance difference is significantly higher than the control group, and after treatment; After experimental treatment of calcitonin original and c-reactive protein, respectively （50. 68±104） ng/ml and （1.57±21.41）mg/L, were significantly lower than before treatment group, and after treatment with comparative no significant differences. CONCLUSION For the treatment of the effect not good, antibiotics or hormone use time is long, COPD patients are using breathing machines, medical personnel should consider some factors, and corresponding inspection to timely find fungal infections and therapy.
