糖尿病合并脑梗死老年患者医院感染的高危因素与干预措施

摘　　要：目的探讨糖尿病(DM)合并脑梗死老年患者医院感染的高危因素和干预措施。方法回顾性分析720例DM合并脑梗死患者的临床资料,根据有无医院感染将患者分为感染组和非感染组;同时选取同期DM非脑梗死医院感染老年患者96例为对照组;比较各组参数异同。结果感染组患者年龄(71.68±8.93)岁、住院天数(31.08±11.21)d、空腹血糖(13.62±4.11)mmol/L、糖化血红蛋白A1C(14.85±5.08)、病程(5.03±4.19)d、侵入性操作、抗菌药物联合应用和免疫制剂及激素使用比例显著高于非感染组(P<0.05);感染组住院天数(31.08±11.21)d、侵入性操作比例显著高于对照组(P<0.05)。结论 DM合并脑梗死患者发生医院感染与年龄、住院天数、空腹血浆葡萄糖、糖化血红蛋白A1C、病程、侵入性操作、抗菌药联合和免疫制剂及激素应用有关;采取相应干预措施及控制危险因素可降低医院感染发生率。OBJECTIVE To investigate the high risk factors and preventive measures of nosocomial infection in gerontal diabetes mellitus patients with cerebral infarction. METHODS Seven hundred and twenty gerontal diabetes mellitus patients with cerebral infarction, were analyzed retrospectively. All patients was divided into infection and non-infection group. Meanwhile, We also selected 96 gerontal diabetes mellitu （no cerebral infarction） with nosocomial infection as control. The results were compared between the groups. RESULTS Age（71.68 ±8.93）, hospitalized days（31.08±11.21）, FPG（13.62±4.11）, HbAlc（14.85±5.08）, course of diabetes mellitus（5.03 i4.19）, invasive treament procedures（69/72）, application of antibiotics （42/99） were increased significantly in infection group compared to those of non-infection group （68.54±10.37, 21.34±9.96, 12.27±3.82, 13.57± 4.63, 4.24±4.06, 102/477 and 116/463, P〈0.05）. Hospitalized days（31.08±11.21） and invasive treament procedures （69/72） were increased significantly in infection group compared to those of control group（27.48 ± 9.98 and 31/65, P〈0.05）. CONCLUSION Preventing and controlling the risk factors can reduce the rate of nosocomial infection among the DM patients with cerebral infarction.
