糖尿病合并尿路感染的临床特点观察

摘　　要：目的探讨糖尿病患者合并尿路感染的临床特点。方法回顾性分析2008年7月-2010年7月收治的1180例糖尿病住院患者临床资料。结果糖尿病合并尿路感染260例,感染率为22.03%,存在典型尿路刺激症状140例;210例进行尿培养,分离出病原菌160株,其中革兰阴性杆菌80株、革兰阳性球菌58株、真菌22株;药敏试验大肠埃希菌对庆大霉素、氨苄西林耐药率为85.1%～94.2%,对头孢哌酮、头孢曲松耐药率较低,为8.4%、16.4%;革兰阳性球菌对万古霉素耐药率为3.8%;年龄大、女性、糖化血红蛋白水平高、存在并发症、住院时间长、预防应用抗菌药物是感染的危险因素;尿路感染组死亡率3.85%高于无尿路感染组的1.83%。结论糖尿病发生尿路感染难以完全避免,感染症状部分不典型,造成病情加重;因此,应积极控制血糖,加强支持疗法,定期查尿常规,避免应用抗菌药物;缩短住院时间。OBJECTIVE To investigate the clinical features of diabetes patients with urinary infection. METHODS The clinical data of 1180 cases of diabetic patients hospitalized from Jul 2008 to Jul 2010 were retrospectively analyzed. RESULTS Among 260 patients with diabetes mellitus and urinary tract infections （22.03%）, 140 cases of typically urinary irritation symptoms, 210 cases were performed the urine culture. 160 strains were isolated： 80 cases of Gram-negative bacteria, 58 strains for Gram-positive cocci, 22 cases of fungi, the drug resistant rates of E. coli to penicillin, ampicillin and gentamycin were 85. 1%-94. 2%, and which to cefoperazone ceftriaxone sodium were with the low resistance, 8.4 %, 16.4 V0, the resistant rate of Gram-positive cocci＇ to vancomycin was 3.8%, Female, age, HbAcl high, complications, the length of time, prevent antibiotics is the risk of infection factors, Urinary infection is 3.85% group, mortality than 1.83% group of urinary tract infection. CONCLUSION Diabetes is hard to completely avoid the urinary infection, and is not a typical symptoms of illness. Therefore, it should actively control the blood sugar, strengthen supportive therapy, the regular check of routine urine to avoid antibiotics and shorten the hospital stay.
