摘 要:目的探讨新疆塔克拉玛干"沙漠人"早期肾脏功能损害的影响因素。方法应用世界卫生组织MONICA方案,随机抽取282例"沙漠人"(沙漠组)与于田县维吾尔族成年人237例(于田组)进行肾脏功能流行病学调查、膳食调查、血液及尿液生化检测等。结果沙漠组普通人群微量白蛋白尿(MAU)检出率10.5%,肾功能减退[肾小球滤过率(GFR)﹤60ml/min]检出率3.88%;高血压人群中两项指标检出率分别为18.75%、9.09%。沙漠组与于田组间收缩压、舒张压、血尿素氮、血尿酸、GFR均有显著性差异(P<0.05);多因素logistic回归分析显示,性别是"沙漠人"MAU的危险因素,低摄盐量为"沙漠人"MAU的保护因素;高龄、收缩压高、舒张压高是"沙漠人"影响GFR的危险因素。结论高龄、性别、高收缩压、高舒张压是"沙漠人"早期肾功能损害的危险因素。"沙漠人"中MAU的检出率偏高,肾功能减退的检出率为3.88%,可能与该地区极端干旱、酷热对人体的影响有关。Objective To investigate the early-kidney damage of "desert people" in the Taklimakan desert of Xinjiang, and to explore the correlation between MAU and some risk factors as well as GFR. Methods The survey was carried out in a natural population by cluster sampling of totally 282 individuals of "desert people", using the method of WHO MONICA. The cross-check analysis between the "desert people" and 237 randomly sampled individuals of Uighur populations in Yutian county of the Hotan area. Results Based on the results of the two independent samples t test, the two groups were significantly different in SBP,DBP,BUN,UA, in blood biochemistry and GFR(P 0.05). Classification of non-conditional binary logistic regression analysis indicated that sex was associated with MAU risk factor,as well as low salt consumption in diet was correlated with MAU protective factor in desert people group. Older age, SBP high and DBP high were associated with GFR risk factors in desert people group. Conclusions Sex, older age, SBP high and DBP high are all correlated with risk factors of early-kidney damage in desert people group. The detection rate of MAU of "desert people" is 10.5%, and the detection rate of GFR that defined less than 60ml/min is 3.88%. And extreme hot environment of the desert draught area may account for its effect on the human body.
