TURP术患者预输霍姆与血中电解质浓度变化

摘　　要：目的探讨经尿道前列腺电切术(TURP)患者预输霍姆与血中电解质浓度变化。方法选择行TURP手术患者80例,随机分成两组,H组为实验组,手术开始后,以5 ml/kg的剂量输入霍姆溶液,15～25 min输完,C组为对照组,术中输平衡液,两组患者均采用腰硬联合麻醉,分别于手术开始输霍姆前(T0)、霍姆输入完毕(T1)、手术结束30 min(T2)抽取对侧肘静脉血检测血钾、血钠、血氯、血钙和血糖,对结果进行分析比较。结果两组间血钠、血氯浓度在T1时差异有统计学意义(P<0.05),T2后恢复相近;H组患者术中未出现经尿道电切综合征(TURS),C组发生1例TURS。结论 TURP术患者预输霍姆能有效地维持血中电解质的稳定,对TURS的发生有一定的预防作用。Objective To research the change of concentration of electrolyte after injecting Holme into the TURP surgery.Methods We choose 80 cases of TURP surgery,which has been divided randomly into two groups,the group H was being as the experimental group,which was injected Holme in dose of 5ml/kg in 15～25 minutes when the surgery beginning,the other group C was being as the control group,which was injected balance solution.Both of the two groups were the epidural anesthesia.We respectively extracted the contralateral cubital vein before injecting Holme（T0）,after injected Holme（T1） and 30 minutes after the surgery（T2） to detect the serum potassium,serum sodium,serum chloride,calcium and glucose,and then analyze the results.Results There were significant differences of the concentration of serum sodium,serum chloride between the two groups in T1（P0.05）,and differences disappear in T2.The TURS in group H does not appear,but one case of TURS appear in group C.Conclusion Injecting Holme into the TURP surgery can effectively maintain the stability of the electrolyte and prevent the occurrence of TURS.
