男男性行为人群的危险行为与社会压力及其影响因素的网络调查

摘　　要：目的了解男男性行为人群（MSM）高危行为特征,面临的社会和心理压力及其影响因素。方法通过网络在线对MSM进行调查,采用非条件Logistic回归分析影响因素。结果共调查838人,年龄（28.73±7.51）岁;自认为性取向为同性恋的占59.07%,双性恋的占40.09%,异性恋者占0.84%。MSM的同性性伴数（中位数）累计有14个（8~31个）女性性伴数累计有2个（1~4个）;最近6个月与同性性交时每次使用安全套的占46.30%。多因素非条件Logistic回归分析表明,文化程度较低、首次性行为年龄较小是影响使用安全套的阻碍因素,曾花钱买性是促成使用安全套的因素。838人中,75.78%认为暴露性取向会明显受到歧视和伤害,19.09%曾因男男性行为有过强烈的自杀念头,11.58%曾因男男性行为有自杀行为,47.02%曾因同性性倾向感到痛苦并影响了学习或工作,21.43%曾因男男性行为暴露被单位辞退或自己辞职。多因素非条件Logistic回归分析表明,结过婚是增加MSM社会压力及心理压力的因素,而文化程度较低是减轻压力的因素。233人检测过艾滋病病毒（HIV）,阳性率为11.59%。结论 MSM存在较高的高危行为,面临着巨大的社会压力和心理压力,HIV感染率较高。Objective To understand HIV/AIDS related high risk behaviors,social and psychological pressures and their influencing factors among MSM.Methods An anonymous online survey was conducted in April of 2010 via the network platform among MSM.Non-conditional Logistic regression was used to analyze influencing factors.Results A total of 838 MSM were surveyed via questionnaires with an average age of 28.73 ±7.51 years,The survey showed that self viewed homosexual sexual orientation was 59.07%;bisexual orientation was 40.09%;and heterosexual orientation was 0.84%.The cumulative number of male-sexual partners were 14（8-31）,and the cumulative number of female-sexual partners were 2（1-4）.The proportion of consistent use of condoms in the last homosexual intercourse was 46.30%.Multivariate logistic regression analysis showed that the factor negatively influencing consistent condom use among the surveyed MSM was having the first sex at young age and lower educational background,The factor promoting consistent condom use among them included buying sex.Of the surveyed MSM,75.78% thought that self exposure of sexual orientation will lead to significant discrimination and enjury;19.09% had strong suicidal intention,11.58% had suicidal attempts,47.02% felt grieved for their homosexual sexual orientation,negatively effecting their learning or work,21.43% were fired or resigned because of exposure of their homosexual orientation.Non-conditional logistic regression analysis showed that marriage was a factor to increase their social pressure and psychological stress,while low education was a factor to reduce the stress.The HIV testing among 233 MSM showed that the positive rate was 11.59%.Conclusion The survey has shown that MSM have high risk sexual behaviors and are faced with enormous social pressure and psychological stress.
