老年糖尿病患者干预研究

摘　　要：目的对老年糖尿病患者的生活方式、多种心血管疾病危险因素进行干预,预防心血管事件的发生、发展。方法对257例老年糖尿病患者的生活方式、高血糖、高血压、血脂异常、体质指数等危险因素进行干预,历时4年,定期监测、随访。结果基线时,糖尿病患者合并心血管疾病情况为高血压68.09%,冠心病57.59%,脑卒中20.23%,急性心肌梗死8.17%。干预后,吸烟率下降54.88%,肥胖者减少14.74%(P<0.05);空腹血糖水平无明显变化;总胆固醇、低密度脂蛋白胆固醇、三酰甘油和血压达标率分别由16.34%、15.95%、49.42%和37.74%上升至59.65%、65.79%、70.18%和60.09%,分别提高了265.06%、312.48%、42.01%和59.22%(P<0.01);脑卒中患病率下降68.71%,主要心血管事件减少54.88%(P<0.05)。结论生活方式与药物治疗相结合的干预措施,能使糖尿病相关危险因素得到有效控制,心脑血管事件发生率明显减少,对预防糖尿病患者心血管疾病的发生、发展有显著效果。Objective To intervene in the lifestyle and multifactors of cardiovascular disease in elderly diabetes patients,in order to prevent the incidence and development of cardiovascular disease. Methods Intervention in the lifestyle, hyperglycosemia, hypertension,dyslipidemia,body mass index of 257 diabetes patients which were followed up for four years were conducted. Results The diabetes patient＇s combined hypertension,coronary heart disease,stroke,acute myocardial infarction were 68.09%,57.59%, 20.23%,and 8.17% respectively at baseline. Compared with baseline levels,smoking rate reduced 54.88%,obesity rate reduced 14.74%（P〈0.05）. The level of fasting blood-glucose getting to optimal state were about 50% during the five years without difference. The rate of TC,LDL-C,TG,and blood pressure reached 59.65%,65.79%,70.18%,and 60.09% from 16.34%,15.95%,49.42%,and 37.74%,increased 265.06%,312.48%,42.01%,and 59.22%（P〈0.01）respectively. The incidence of stroke reduced 68.71%,while the main cardiovascular events reduced 54.88% by the end of the intervention（P〈0.05）. Conclusions Interventions including lifestyle intervention and medicine intervention on diabetes can effectively control cardiovascular disease risk factors associated with diabetes, reducing the incidence of the cardiovascular events.
