胆道积气致腹腔镜胆囊切除术中转开腹10例临床分析

摘　　要：目的:探讨胆道积气对腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)手术的影响。方法:回顾性分析10例胆道积气患者的临床资料。结果:10例胆道积气的胆结石患者做LC手术均中转开腹,术中粘连重并发现7例胆囊内瘘。结论:B超或CT提示胆道积气的胆结石患者作LC手术前要想到腹腔粘连和胆道内瘘可能,术前建议作ERCP或MRCP检查以明确胆道情况,做到术前充分估计,术中根据情况及时中转开腹。Objective： Discussion of LC surgery biliary product gas influence, Methods retrospective analysis, 10 cases biliary product gas the patient＇s clinical data. Results in 10 cases biliary product gas gallstone patients do LC are performed transshipment sigmoidectomy, Intraoperative adhesion heavy and found within 7 cases cystic fistula. Conclusion b - sonography or CT hint biliary product gas gallstone patient LC before surgery should think of celiac adhesion and biliary fistula may, within, Preoperative Suggestions for ERCP or MRCP check to clear biliary situation, Do intraopcratie preoperative fully estimates, according to the circumstance timely transferred to opening appendectomy.
