摘  要:目的:探讨检测静脉导管的多普勒血流频谱异常在孕早期筛选和协助早期诊断胎儿异常的临床意义。方法:于孕11~14周对3586例胎儿进行静脉导管的多普勒血流频谱检测、颈项透明层厚度测量及胎儿结构筛查。结果:共检测到静脉导管的血流频谱异常病例29例,11例进行了染色体检查发现胎儿染色体异常7例,观察至中孕期超声检查胎儿先天性心脏病10例,其他结构异常3例,2例胎死宫内。结论:孕早期彩色多普勒超声检测胎儿静脉导管的频谱异常可作为筛选和协助临床早期诊断胎儿异常尤其是先天性心脏病超声标记。Objective:To investigate the clinical significance of Doppler flow waveform of ductus venosus(DV) in screening of fetal malformation..Methods:Ductus venosus blood flow velocity wave forms and nuchal translucency thickness of 3 586 fetus with gestation of 11~14 weeks were measured with ultrasonography,who were followed-up till birth.Results:29 abnormal Doppler flow waveform of the DV were obtained.11 fetal karyotypes were detected,and 6 fetal karyotypes were abnormal.By following-up ultrasonographic screening,13 abnormalities were identified.There were 10 congenital heart defects of 13 abnormalities,2 fetuses were died at uterus..Conclusion:Doppler ultrasonographic detection of DV blood flow at 11~14 weeks will be useful as a new prospective technique in screening of fetal abnormalities,especially congenital heart defects.
