64层螺旋CT冠状动脉成像中图像质量的影响因素分析

摘　　要：目的分析64层螺旋CT冠状动脉成像(MSCTCA)中影响图像质量的因素。方法 130例临床考虑冠心病患者包括4例支架植入术后,3例冠状动脉搭桥术后及4例安装起搏器者均经MSCTCA。分析了所有患者冠状动脉显示率及影响图像质量的因素。结果 MSCTCA显示了130例的1412个冠状动脉节段,显示率为90.51%(1412/1560)。冠状动脉节段总体可评价率为87.32%(1233/1412),其余179个(12.68%)为不可评价节段,其原因包括心率过快(n=33),心律不齐(n=27),呼吸伪影(n=27),严重钙化(n=23),重建时相选择不当(n=34)以及造影剂充盈不足(n=35)。MSCTCA对严重钙化的诊断准确率为39.13%(9/23)。结论在64层螺旋CT冠状动脉成像中,影响图像质量的因素主要是造影剂充盈不足,重建时相选择不当,心率过快,心律不齐,呼吸伪影以及冠状动脉管壁的严重钙化。Objective To analyze the influencing factors of imaging quality during 64-slice spiral CT coronary angiography（MSCTCA）.Methods A hundred and thirty patients with clinically considered coronary heart disease,who comprised 4 stent implantation later,3 coronary artery bypass graft later and 4 had installed pacemaker underwent MSCTCA evaluation.The visualizing rate of coronary artery segments and the factors influencing the imaging quality were analyzed in all patients.Results MSCTCA showed that there were 1412 of 1560 coronary segments in 130 cases with 90.15% of visualizing rate.The totally assessable rate of coronary segments was 87.32%（1233/1412）,while other 179（12.68%） segments were non-assessable,whose causes included tachycardia（n=33）,arrhythmia（n=27）,respiratory artifacts（n=27）,severe calcification（n=23）,unsuitable selection of reconstruction phase（n=34） and insufficient filling of contrast medium（n=35）.The diagnostic accuracy of MSCTCA for severe calcification of coronary artery walls was 39.13%（9/23）.Conclusion The factors influencing the imaging quality during MSCTCA are mainly insufficient filling of contrast medium,unsuitable selection of reconstruction phase,tachycardia,arrhythmia,respiratory artifacts,and severe calcification of coronary artery walls.
