导丝引导去顶输尿管导管在双J管更换中的应用

摘　　要：目的评价导丝引导去顶输尿管导管在双J管更换中的疗效和安全性。方法回顾性分析本院定期更换双J管患者15例,其中男性9例,女性6例。输尿管膀胱再植术后2例,子宫颈癌晚期致肾积水3例,胃癌晚期致肾积水2例,高龄患者输尿管狭窄6例,腹膜后纤维化2例。结果所有患者均应用本方法更换双J管,术后经腹部平片及随访证实获得成功。随访6～48个月,未出现穿孔等严重并发症,更换双J管后,无严重血尿病例。结论应用导丝引导去顶输尿管导管,在更换双J管前建立输尿管内支架,可以确保更换导管的成功。Objective To evaluate the efficacy and safety of decapitating ureteral catheters with guide-wires in exchange of double-J stents.Methods Fifteen patients（9 males and 6 females） undergone routine exchange of double-J stents were retrospectively analyzed.The ailments of patients included hydromephrosis caused by advanced cervical（3 patients） and gastric（3 patients） carcinomas,ureteral strictures in 6 aged patients and retroperitoneal fibrosis（2 patients）.Results Postoperative plain film of the kidney,ureter and bladder comfirmed that all procedure were successful.During the 6～48 months follow-up,no severe complications such as perforation and hematuria were found.Conclusion The decapitating ureteral catheters with guide-wires can provide a reliable internal drainage in ureter,thus ensuring success of the exchange of double-J stents.
