益气温阳法结合西药对充血性心力衰竭患者临床症状及心功能的影响

摘　　要：目的观察益气温阳法对充血性心力衰竭临床症状及心功能的影响。方法将60例充血性心力衰竭患者随机分为两组,对照组(30例)予雅施达、倍他乐克治疗,治疗组(30例)在对照组用药基础上予益气温阳中药,两组均治疗14天。观察中医证候疗效、Lee心衰疗效和NYHA心功能疗效,治疗前后分别检测B型脑钠肽(治疗7天)、左室射血分数(LVEF)和心肌耗氧量。结果治疗组中医证候疗效、Lee心衰疗效、NYHA心功能疗效总有效率分别为80.00%、56.67%和86.67%,对照组为53.33%、53.33%和60.00%;组间比较,中医证候疗效和NYHA心功能疗效差异有统计学意义(P<0.05)。治疗后治疗组B型脑钠肽水平、心肌耗氧量和LVEF与治疗前比较差异均有统计学意义(P<0.01);组间治疗后比较,B型脑钠肽水平差异有统计学意义(P<0.05)。结论益气温阳法能有效缓解慢性充血性心力衰竭患者的临床症状,改善心功能,并降低B型脑钠肽水平。Objective To observe the effects of qi-boosting and yang-warming method plus conventional western medicine on clinical symptoms and heart function in patients with congestive heart failure.Methods Sixty patients with congestive heart failure were randomly divided into two groups.All of the patients were treated with perindopril and metoprolol,in addition those in the treatment group were given qi-boosting and yang-warming herbs,with a course of 14 days.Results The overall effective rates for clinical efficacy,Lee CHF and NYHA functional capacity were respectively 80.00%,56.67% and 86.67% in the treatment group,and 53.33%,53.33% and 60.00% in the control group;there were significant differences in the clinical efficacy and NYHA functional capacity between the two groups（P〈0.05）.After treatment,the B-type natriuretic peptide（BNP）,myocardial oxygen consumption and left ventricular ejection fraction（LVEF） were improved in the treatment group（P0.01）,and there was a difference in the level of BNP between the treatment and control groups（P〈0.05）.Conclusion Qi-boosting and yang-warming method is effective in improving clinical symptoms and heart function,and reducing BNP in the treatment of congestive heart failure.
