36例法洛氏四联症超声诊断与造影、手术的对比分析

摘　　要：目的探讨超声心动图诊断法洛氏四联症(tetralogy of Fallot,TOF)的价值。方法回顾性分析36例经手术或心血管造影证实的TOF的超声表现与手术结果。结果 36例患者中超声与手术结果完全符合33例,部分符合1例,漏诊1例,误诊1例,诊断符合率94.44%,漏误诊2例(5.56%);单纯右室流出道狭窄28例,占80.0%(28/35);室间隔缺损多为嵴下型,占71.42%(25/35);超声测值与术中测值的统计学处理结果显示二者相关程度高(r=0.779,P<0.001)。结论超声对于绝大多数TOF具有确诊价值,对个别病例需要注意鉴别诊断。Objective To evaluate the value of ultrasonography in the diagnosis of tetralogy of Fallot（TOF）.Methods A retrospective analysis was performed in 36 patients with TOF,and the ultrasonic appearance was correlated with surgical findings and（or）cardiac angiography findings.Results All cases were confirmed by operation and（or）cardiac angiography.There were 33 cases of absolute coincidence,1 case of partial coincidence,1 case of missed diagnosis,1 case of misdiagnosis.The total coincident rate was 94.44%,the misdiagnosis and missed diagnosis rate was 5.56%（2/36）.Twenty patients was merely right ventricular outflow tract stenosis in 35 patients of definite TOF,the ratio was 80.0%（28/35）.The majority of ventricular septal defect（VSD）was perimembranous defects,the ratio was 71.42%（25/35）.Compared ultrasonographic measure value with surgical measure value,the difference had statistical significance（r=0.779,P0.001）.Conclusion Ultrasonography can contribute to the differential diagnosis of most tetralogy of Fallot.
