过敏性紫癜肾炎患儿血清脂联素和超敏C反应蛋白的变化及意义

摘　　要：目的探讨过敏性紫癜肾炎(HSPN)患儿血清脂联素(APN)、超敏C-反应蛋白(hs-CRP)的水平变化及其临床意义。方法分别采用酶联免疫吸附法(ELISA)和免疫透射比浊法测定44例HSPN患儿血清中APN、hs-CRP的水平,与20例对照组比较分析,并对血清中APN与hs-CRP的水平进行相关分析。结果 HSPN患儿急性期组与恢复期组血清APN的水平均低于对照组(P<0.01),且急性期组血清APN的水平也低于恢复期组(P<0.01);HSPN患儿急性期组与恢复期组血清hs-CRP的水平均高于对照组(P<0.01),且急性期组血清hs-CRP的水平也高于恢复期组(P<0.01);血清APN与hs-CRP的水平呈负相关(r=-0.43,P<0.05)。结论 HSPN患儿不但急性期体内存在有炎症反应,而且恢复期仍存在有轻度的炎症;血清APN参与了HSPN的病理过程,并在HSPN的发生、发展中起着重要作用。Objective To explore the change and clinical significance of serum APN and hs-CRP in the children with Henoch-Schonlein purpura nephritis（HSPN）.Methods The levels of serum APN and hs-CRP in 44 children with HSPN and 20 controls were measured by ELISA and immune nephelonretry.The correlation analysis of the levels of APN and hs-CRP in the children with HSPN were performed.Results The levels of serum APN in the acute and restoration period in the children with HSPN were lower than that of the control group（P0.01）,the level of serum APN in the acute period group was lower than that of the restoration period group（P0.01）.The levels of serum hs-CRP in the acute and restoration period in the children with HSPN were higher than that of the control group（P0.01）,the level of serum hs-CRP in the acute period group was higher than that of the restoration period group（P0.01）.The level of serum APN in the children with HSPN was negatively correlated with hs-CRP.Conclusion There was a mild inflammation reaction in restoration period in the children with HSPN;APN may be involved in the pathogenesis of HSPN,and plays important roles in the occurrence and development of HSPN.
