干燥综合征血液系统损害与免疫学及各临床指标的相关性分析

摘　　要：目的探讨原发性干燥综合征(PSS)患者的临床特点与各临床实验指标的相关性。方法分析97例PSS患者的临床资料,合并肺间质病变、甲状腺疾病的情况及血常规、抗核抗体、抗SSA、抗SSB抗体等检测结果,分析白细胞减低PSS与非白细胞减低PSS患者各临床指标的不同及与其相关性。结果 97例PSS患者中出现血液系统损害51例(52.6%),白细胞减少47例(48.5%),贫血28例(28.9%),血小板减少6例(6.2%)。47例白细胞减低PSS患者抗SSA、抗SSB抗体阳性率(31例,65.9%;18例,38.3%)高于无血液受损组(22例,44.0%;12例,24.0%);组间比较,两组间抗SSA抗体阳性率差异有统计学意义(P<0.05)。结论抗SSA抗体可能与PSS的血液系统损害,特别是白细胞减少相关。Objective To investigate the relationship between hematologic abnormalities such as leukopenia and clinical parameters in primary Sjogren＇s syndrome（PSS）.Methods The clinical parameters of pulmonary fibrosis,thyroid gland involvement of leukopenia and immunologic markers such as ANA,AntiRo（SSA）and antiLa（SSB）antibodies and complete blood counts in 97 patients with PSS were analyzed.Results The positive incidence of SSA in patients with leukopenia was significantly higher than that in patients with normal blood cells（P〈0.05）,the incidences of IgG or RF elevation and pulmonary fibrosis or thyroid gland involvement in the leukopenia group were much higher than those in control group,but no statistical difference was found.Conclusion AntiRo（SSA）antibodies may be closely associated with the hematologic involvement,especially withleukopenia,which could be helpful in estimating the pathogenetic condition and prognosis of PSS patients.
