摘  要:目的观察慢病毒载体介导的BDNF与电针对神经病理痛模型大鼠热辐射痛阈的影响差别,比较基因与电针的镇痛疗效,探讨BDNF对疼痛可能的作用机理。方法本研究采用坐骨神经结扎损伤的神经病理性疼痛大鼠CCI模型,运用荧光免疫标记法,观察慢病毒载体介导的BDNF与电针干预后大鼠不同时间点(3 d、7 d、21 d)BDNF在相应节段脊髓内表达的变化。另运用BME-410C热痛刺激仪记录大鼠术前(0 d)及术后1 d、3 d、5 d、7 d、9 d、14 d、21 d,各组的热辐射刺激缩足潜伏期痛阈的变化。结果大鼠热痛敏阈值的变化:各组大鼠CCI术前热痛敏阈值均无明显差异。模拟对照组大鼠各时点热痛敏阈值无明显差异。阴性对照组与空白对照组各时点相比无统计学差异。CCI术后一天各组热痛敏阈值低于模拟对照组(P<0.05)。术后第5天,试验组、电针组热痛敏阈值上升,高于空白及阴性对照组(P<0.05),术后7天,实验组上升至最高点,试验组与电针组无明显差异(P>0.05)。术后9天、14天、21天,实验组热痛敏阈值下降,术后7天、9天、14天电针组热痛敏阙值趋于稳定,均高于空白及阴性对照组(P<0.05),术后9天、14天实验组与电针组有明显差异(P<0.05),术后21天,试验组与电针组无明显差异(P>0.05)。荧光免疫组化观察:空白对照组、阴性对照组及模拟对照组术后第3天、第7天、第21天时点大鼠L4/5段脊髓阳性神经元表达无明显变化(P>0.05),试验组三时点阳性神经元先增加后降低,术后第7天最高,与其他组相比阳性神经元表达明显升高(P<0.05),电针组阳性神经元有所增加,但与阴性对照组无明显差异(P>0.05)。结论 BDNF参与了大鼠痛敏的过程。可能外源性BDNF表达有利于受损神经修复。Objective To observe the different effects of heat emission pain threshold between lentiviral vector mediated BDNF and electroacupuncture on neuropathic pain model for rat,comparing gene and electroacupuncture analgesic effect,studying the possible mechanism of BDNF for pain treatment,as a result,it provided scientific experimental data for opening up new aeras of curving intractable neuropathic pain.Methods In this study the neuropathic pain of sciatic constriction injury CCI model for rat was adopted.Immunofluorescence labeling was used.Expression pattern of BDNF in the correspondent spinal cord was observed By investigating the lentiviral vector mediated BDNF and electroacupuncture intervention changes of rats at several time points(3 d、7 d、21 d).Meanwhile,by BME-410C thermal pain stimulation system the changes of different groups of thermal radiation paw withdrawal thermal latency(PWTL) was measured before(0 d) and at 1 d、3 d、5 d、7 d、9 d、14 d、21 d after operation.Results Changes of paw withdraw thermal latency for rat:There was no significant difference for all groups of rats CCI paw withdraw thermal latency before operation.The paw withdraw thermal latency of simulated group for rats were almost the same.There was no significant difference between negative group and control group.The treatment groups of paw withdraw thermal latency after the first day CCI operation ware lower than the simulated control group(P0.05).On the fifth day after operation,the model group and electro-acupuncture group of paw withdraw thermal latency rose,higher than control group and negative group(P0.05).On the seventh day,the model group increased to the highest point.There was no significant difference between model group and electro-acupuncture group(P0.05).On the 9 th day,14 th day,21 st day after operation the model group reduced,and on the 7 th day,9 th day,14 th day,the electro-acupuncture group was tending towards stability.They were both higher than control group and negative group�
