BiPAP呼吸机治疗COPD合并Ⅱ型呼吸衰竭疗效观察

摘　　要：目的探讨BiPAP（双水平正压通气）呼吸机在治疗慢性阻塞性肺疾病（COPD）合并Ⅱ型呼吸衰竭中的价值。方法在给予吸氧、抗感染、呼吸兴奋剂等常规治疗的基础上，加用BiPAP呼吸机通气治疗，观察COPD合并Ⅱ型呼吸衰竭患者治疗前后pH值、PO2、PCO2、呼吸频率、心率等指标的变化。结果34例COPD合并Ⅱ型呼吸衰竭患者中29例加用Bi-PAP无创呼吸机通气，治疗后pH值、PO2、PCO2、呼吸频率、心率等指标变化有显著性改善（P〈0．01）。结论应用BiPAP呼吸机治疗COPD合并Ⅱ型呼吸衰竭的疗效满意、并发症少，患者易于接受。Objective To evaluate the effect of Bi-level positive airway pressure （BiPAP） ventilator on chronic obstructive pulmonary disease （COPD） patients complicated with type II respiratory failure. Methods On the basis of routine treatment such as oxygen, antibiotics, respiratory stimulant, BiPAP ventilator was used for 34 COPD patients complicated with type II respiratory failure and the changes of their clinical manifestation were observed, including blood gas parameters PH, PO2, PCO2, and respiratory rate, heart rate. Results In the 34 patients, there were 29 patients＇clinical manifestation were obviously improved, the values of PH and PO2 were higher, the PCO2 , respiratory rate and heart rate were lower. The differences on the clinical manifestation of the patients before and after the using of BiPAP ventilator were significantly difference （P 〈 0.01 ）. Conclusion BiPAP ventilator is effective and safe in the treatment of COPD patients complicated with type II respiratory failure.
