湖南省四种不同农村卫生监督模式的县级卫生监督机构2006-2008年经费来源分析

摘　　要：目的评价2006-2008年不同农村卫生监督模式的县级卫生监督机构经费来源情况,为健全基层卫生监督体系提供科学依据。方法采用分层抽样的方法,用调查表调查12个样本县卫生监督机构2006-2008年卫生监督经费来源情况。结果四种不同模式各模式总平均卫生监督经费以财政拨款所占比例最大,占53.5%;服务收入次之,占43.6%;2006-2008年四种模式间的人均财政拨款均为增长趋势;不同模式间人均财政拨款差异有统计学意义(F=23.74,P=0.00)。2006-2008年专项经费来源于中央(即中西部项目)、省、市(州)、县级拨款所占比例分别为10.0%、22.8%、25.5%、41.7%,且四种不同模式的专项经费拨款来源构成差异有统计学意义(F=3.97,P=0.035)。结论 2006-2008年四种模式间的人均财政拨款均为增长趋势,但服务收入所占比例超过40%,与卫生监督机构的公益性质相矛盾。建议政府增大财政投入,优化卫生监督经费渠道。Objective To compare the 4 different modes of funding sources in prefectural health supervision stations,and to provide the scientific evidence for establishing and improving the system of grass-roots health supervision stations.Methods Using stratified variables,the funding sources of 12 sample-prefectural health supervision stations during the period of 2006～2008 were investigated through questionnaires.Results Among the four different modes of funding sources for health supervision,funds that allocated from the State Treasury accounted for 53.5% of the funding,followed by service revenue（43.6%）.In the four different modes,per capita funds that allocated from the State Treasury during the period of 2006～2008 showed an increasing trend.There were statistically significant differences in per capita funds that allocated from the State Treasury among the four different modes（F=23.74,P=0.00）.The ratios of special funds sources from central（Midwest project）,provincial,municipal,and prefectural governments during the period of 2006～2008 were 10%,22.8%,25.5%,and 41.7% respectively,and moreover,the difference was statistically significant in special funds sources among the 4 different modes（F=3.97,P=0.035）.Conclusions Per capita funds allocated from State treasury in the four different modes show a trend in growth,however the percentage of service revenue surpassed 40% which is contradictory to the public interest of health supervision stations.This suggests that fiscal revenue should be enhanced,while at the same time the channel of health supervision should be improved.
