湖南省2004-2010年孕产妇HIV检测情况分析

摘　　要：目的了解湖南省孕产妇艾滋病病毒(HIV)感染状况,为开展预防艾滋病母婴传播工作提供决策依据。方法对2004年1月-2010年12月湖南省(2004-2009年8月30日为6个示范区数据,2009年9月-2010年12月为全省数据)孕产妇的HIV检测结果进行分析。结果 2004-2010年共筛查孕产妇1 223 557人,确认HIV抗体阳性者155例,总体阳性率为1.97/万。155例HIV感染孕产妇中产前确诊者98例,占63.2%;产时确诊者21例,占13.5%;产后确诊者36例,占23.3%。自愿选择终止妊娠41例,占26.5%;103例选择继续妊娠直至分娩,占66.5%,分娩的产妇中服用抗病毒药物者30例,占29.1%;其中采用三联用药者22例,占73.3%。另7例正处于孕期随访中;2人失访。103例分娩的婴儿中均为活产,其中3例出生后7 d内死亡,存活的100例婴儿中服用抗病毒药物者36例,占36.0%。结论加强孕产妇HIV检测,提高孕早期HIV检测率,加强对HIV感染孕产妇的高危管理,可以提高湖南省预防艾滋病母婴传播服务水平,降低艾滋病母婴传播率。Objective To investigate the status of HIV infection among pregnant women in Hunan Province,and to provide a scientific basis for preventing mother-to-child transmission of HIV（PMTCT）.Methods The HIV test results of pregnant women in Hunan Province from January 2004 to December 2010 were collected and analyzed.Results A total of 1,223,557 pregnant women were tested for HIV antibody from 2004 to 2010,of whom 155 pregnant women were diagnosed with HIV positive and the HIV-positive rate was 1.97/10,000.The 155 HIV-positive women were diagnosed in different period with 98（63.2%） antenatal,21（13.5%） intrapartum and 36（23.3%） postnatal tested.Of the 155 HIV-positive pregnant women,41（26.5%） voluntarily decided to terminate pregnancy,103（66.5%） chose to continue with pregnancy until delivery and 7 were observed in pregnancy.30（29.1%） of the 103 delivered women took PMTCT antiviral medicine in which 22（73.3%） adopted trigeminal scheme.103 babies were born alive,of whom 3 died in 7 days after birth and 36 babies were provided with antiviral medicine.Conclusions The PMTCT service level can be improved and the rate of mother-to-child transmission of HIV can be decreased by promoting HIV-testing among pregnant women,especially in the early period of pregnancy and enhancing the high dangerous management of the HIV-positive pregnant women.
