股骨转子间骨折手术方法的选择

摘　　要：目的探讨股骨转子间骨折的特点及合适的手术方法。方法股骨转子间骨折85例,动力髋螺钉(DHS)固定55例,锁定加压钢板固定9例,人工股骨头置换21例。结果经术后2个月至3年随访,DHS内固定组4例出现轻度髋内翻畸形;锁定钢板组1例轻度髋内翻畸形。按黄公怡关节功能评价标准评价,优48例,良29例,可7例,差1例。优良率90.1%。结论治疗股骨转子间骨折,应根据患者的全身情况,骨折类型,选择合适的手术方法。Objective To explore the significance of internal fixations of femoral intertrochanteric fractures.Methods Internal fixation was performed with the DHS（55 patients）.Locking plate（9 patients）,Joint replacement（21 patients）for the 85 cases of femoral Intertrochanteric Fractures.Results 85 patients were followed up from 2 months to 3 years after the operation.There were 4 cases deformity of hip in the dynamic hip screw group.There was 1 case deformity of hip in the locking plate group.The results were excellent in 48 patients,good in 29 patients,fair in 7,and poor in 1,according to Huang Gongyi rating scores,with 90.1% of excellent and good functions of hip.Conclusion They are all effective options to solve the femoral intertrochanteric fractures.The appropriate one should be selected according to the patient＇s age and the type of fracture.
