农村老年肺结核患者发现延迟对医疗费用的影响因素研究

摘　　要：目的了解农村老年肺结核患者发现延迟期浪费的医疗费用,分析发现延迟期医疗费用的影响因素。方法采取横断面调查方法,除采用统一的专用门诊病历外,还运用自制的个案调查表,对农村老年新发涂阳肺结核患者进行调查。结果从症状出现到被确诊为肺结核期间医疗费用的中位数为190元;发现延迟期医疗费用的影响因素有:首诊结果、确诊前就诊次数、是否住院、首诊单位、是否有确诊延误。结论农村地区老年肺结核患者发现延迟期浪费的医疗费用较高。需减少确诊延误,控制住院患者数并加强农村老年人群结核病健康教育以减少农村老年肺结核患者在发现延迟期的医疗费用。Objective To investigate the influence factors of medical expense of diagnostic delay in elderly rural tuberculosis patients and provide a scientific basis for prevention and control of pulmonary tuberculosis in elderly patients.Methods A cross-sectional survey was conducted in the elderly with new smear positive tuberculosis living in rural areas,by using the unified special-purpose outpatient service medical records as well as self-designed questionnaire.Results The median of medical expense of diagnostic delay in the elderly rural tuberculosis patients was 190 yuan.The analysis results showed that factors significantly contributed to medical expense of diagnostic delay were the status of primarily diagnosis,the number of visits before diagnosed,hospitalization or not,the medical institution first visited and the diagnostic delay.Conclusion Medical expense of elderly tuberculosis patients with diagnostic delay in rural areas was high,for this issue,such as,reducing diagnostic delay,controlling the pulmonary patients in hospital and strengthening health education among the elderly in rural areas should be taken in order to reduce the medical expense.
