摘 要:【目的】研究Smac(Second mitochondria—derived activator of caspases)基因在大动脉转位伴室间隔缺损(D-TGA/VSD)手术前后表达水平的差异与围手术期心功能改变的相关性;以探求体外循环引起心功能损伤的分子生物学机制与心肌保护的新方法。【方法】实时荧光定量PCR检测Smac基因在先天性心脏病手术前后表达差异,并与手术前后心功能的变化行相关回归分析。【结果Smac基因在D-TGA/VSD术后的表达高于术前(P〈O.05);且该基因在手术前后表达差异与主动脉阻断时间,术后射血分数(EF)、缩短分数(FS)改变有相关性(r。分别为0.736,0.695,0.531)。【结论】在INTGA/VSD的治疗中,长时间体外循环引起’Smac基因高表达,进一步导致术后心功能的下降。这一现象是否存在与其他复杂性先天性心脏病的治疗中,尚有待证实。[Objective] To research the correlation between the expression difference of second mitochon- dria-derived activator of caspases(Smac) gene and cardiac function change in the peroperative period of the op- eration of transposition of great artery with ventricular septal defect(D-TGA/VSD) in order to find the molec- ular biological mechanism of cardiac function damage caused by extracorporeal circulation and the new method of myocardial protection. [Methods] Real-time fluorescent quantitative PCR was used to measure the expres- sion difference of Smac gene before and after the operation of congenital heart disease. The correlation regres- sion analysis was carried out for cardiac function changes and expression difference of Smac gene before and af- ter the operation. ~Resultsl The expression of Smac gene after the operation of D-TGA/VSD was higher than that before operation( P 〈0.05). The expression difference of Smac gene between before and after operation was correlated with the aortic clamping time and the level of EF and FS after operation(rs =0. 736, 0. 695, 0. 531). [Conclusion] During the treatment of D-TGA/VSD, long period of extracorporeal circulation can cause the high expression of Smac gene, and further lead to the deterioration of cardiac function after the operation. Whether such phenomenon exists in the other treatments of the complicated congenital heart diseases is to be verified.
