摘　　要：【目的】研究应用右美托咪定（Dex）对老年人七氟醚最低肺泡有效浓度（MAC）的影响。【方法140例65岁以上全麻下择期行腹部手术老年患者，随机分为两组。A组（n=20）诱导前静脉泵入Dex负荷剂量1μg／kg，时间超过10min，维持剂量O．4t,g／（kg·h），稳定后吸入七氟醚诱导；B组（n=20）泵入等量生理盐水，然后吸入七氟醚诱导麻醉。观察各组患者对切皮刺激的反应。采用序贯法计算七氟醚的MAc。【结果】A组MAC的ED5。为2．85％，95％可信区间为2．59％～3．07％；B组MAC的ED5。为4．26％，95％可信区间为4．04％～4．49％，两组相比有统计学差异（P〈0．05）。【结论】老年人应用Dex能有效降低七氟醚的MAC值，负荷剂量1ttg／kg，维持剂量0．4,ug／（kg·h）能降低MAC33％。[ObjectivelTo investigate the effect of dexmedetomidine on the minimum alveolar concentration （MAC） of sevoflurane in elderly patients. KMethodsl Forty patients aged over 65 years old who underwent elective abdominal surgery with general anesthesia were analyzed. Before anesthesia induction, group A（ n =20） was giv- en dexmedetomidine with the loading dose of lug/kg for more than 10min and with the maintenance dose of 0.4 /μg/（kg h）, and then inhaled sevoflurane for induction. Group B（ n = 20） was given the equal amount of normal saline, and then inhaled sevoflurane for induction. The response to skin incision in two groups was observed. The MAC of sevoflurane was determined by using sequential method, [Results] The ED50 of MAC was 2.85 % （95 % confidence interval ranged from 2.59% to 3.07%） in group A and 4.26% in group B（95% confidence interval ranged from 4.04 % to 4.49 %）. There was significant difference between two groups（ P 〈0.05）, [Conclusion] Dexmedetomidine can effectively decrease the MAC of sevoflurane in elderly patients by 33 % with loading dose of 1 μg/kg and maintenance dose of 0.4 /μg/（kg h）.
