摘　　要：目的探讨颅内动脉瘤显微夹闭手术中破裂的预防及处理的技巧和方法。方法回顾性分析2002年2月～2010年8月55例患者开颅行夹闭术中动脉瘤破裂8例的处理,其中后交通2例,脉络膜前2例,大脑中分叉1例,前交通3例。结果 8例术中破裂动脉瘤均被成功夹闭,其中1例死亡,2例中残,1例重残,余恢复良好。结论颅内动脉瘤夹闭术中的破裂难以避免,显微手术技巧及综合处理措施能预防或减少术中破裂,有效地改善患者预后。Objective To explore the prevention, microsurgical technique, and comprehensive measures of intracranial ruptured second time during operation. Methods A retrospective analysis was made 55 patients.Of 8 aneurysms ruptured second time during operation operated on from February 2002 to Augest 2010.Two posterior communicating artery aneurysm.Two choroid before artery aneurysms, 1 middle cerebral artery aneurysms, 3 anterior communicating artery aneurysms.Results 8 ruptured aneurysms （luring operation were clipped successfully.During 3 case were good recovery, mild disability in 2 cases, serious disabibity in 1 case, and 1 death.Conclusion Intracranial aneurysmal rupture during clip operations may be impossibly avoided.It seems possible to reduce incidence of intraoperative aneurismal rupture and improve the prognosis by utilizing microsurgical and comprehensive measures.
