肌皮神经的解剖及其临床意义

摘　　要：目的对肌皮神经位置及分支进行观测,为临床肌皮神经损伤与神经移位修复提供解剖学基础。方法对15具成人防腐尸体的双侧上肢进行解剖观察,并测量肌皮神经的起点、长度、分支及交通支等。结果肌皮神经主干长(50.07±46.08)mm,起始点左右径(3.21±1.17)mm,前后径(1.97±1.16)mm,起点距喙突距离为(41.89±13.55)mm,肌皮神经穿喙肱肌者占92.86%(26侧)。共发现五侧上肢存在肌皮神经至正中神经的交通支。结论肌皮神经走行、起点和分支的变异均较大,并有交通支存在,临床上应注意这些变异,以便在修复肌皮神经损伤时选择最佳术式和避免不必要的额外损伤。Objective To provide anatomy knowledge for musculocutaneous nerve clinical restoration and transportation by observation of its location and its branches.Methods We dissected the bilateral forearms of 15 adult cadavers,determined the origins of musculocutaneous nerve,measured its length and studied its branches and communicating branches as well.Results The musculocutaneous nerve trunk was（50.07 ± 46.08） mm long.It starts at（41.89 ± 13.55） mm from coracoid process.The left-right diameter of the nerve origin was（3.21 ± 1.17）mm,and its front-back diameter was（1.97 ± 1.16）mm.There were 92.86%（26 case musculocutaneous nerve though coracobrachialis.We also observed 5 case of communicating branches between musculocutaneous nerve and median nerve.Conclusion There are great variation in musculocutaneous nerve＇s origin,passage and branches with communicating branches with other nerves.It is very important to know these variations,in case of extra damage to musculocutaneous nerve when trying to repair it.This study can also provide voluble information to the selection of operation method in musculocutaneous nerve restoration.
