摘 要:目的探讨计算机辅助设计个性化颅骨修补术在额颞顶颅骨缺损治疗中的临床价值。方法对自2008年4月-2011年4月神经外科使用计算机辅助个性化颅骨材料进行额颞顶颅骨修补术76例病例进行分析。术前头颅薄层CT,使用计算机辅助的数字化个体塑形技术对材料进行塑形,经过患者认可后选择材料制作补片,进行颅骨缺损修补术。结果 76例额颞顶颅骨缺损患者均行颅骨修补术,平均手术时间65min,其中6例病例术前行脑室-腹腔分流术,术后并发症:皮下积液6例治疗后消失;继发出血3例:1例硬膜外血肿经手术清除,2例脑内血肿,其中1例少量血肿内科治疗吸收,另一例行血肿清除并去除修补材料;切口感染2例为皮下积脓去除修补材料。结论计算机辅助设计个性化颅骨修补颅骨缺损,能恢复患者原有外形,缩短手术时间,操作简单,具有临床推广价值。Objective To explore the clinical effect of individualized cranioplasty of computer-assisted design in repairing skull defects. Methods Form Apr. 2008 to Apr. 2011,76 patients with skull defect were analyzed retrospectively, uhrathin cranial CT scan was performed before operation, the templates of skull defect were made under computer assistance. After patient' s consent, the skull defect patches were manufactured with titanium-networks and were implanted by the operation. Results 76 patients were treated with skull repairing, the average operation duration was 65 minutes, among them ,6 patients underwent ventriculo-per- itoneal shunt before skull repairing. Postoperative complications were as follows, collection of fluid under the scalp was found in 6 eases, secondary bleeding in 3 cases, incision infection in 2 cases. Conclusion Individualized cranioplasty of computer-assisted design in repairing skull defects can restore patient' s configuration, shorten the operation time, make operation easily, and had clinical spread value.
