摘  要:目的:观察利多卡因预处理对局灶性脑缺血再灌注大鼠脑组织水通道蛋白4(AQP4)表达的影响,探讨其脑保护作用机制。方法:96只雄性SD大鼠随机分成假手术组(sham)、缺血组(ischemia)、利多卡因组(lidocaine)及生理盐水组(saline)(缺血前30 min腹腔注射利多卡因10 mg.kg-1及同等体积的生理盐水),采用大脑中动脉线栓法建立局灶性脑缺血模型,缺血2 h再灌注72 h行神经功能缺损评分、断头取脑行脑梗死体积、脑含水量、脑组织内伊文蓝含量测定,蛋白印迹法行脑内AQP4表达检测。结果:利多卡因组神经功能缺损评分、脑梗死体积、脑含水量及伊文蓝含量均较缺血组与生理盐水组低,同时脑内AQP4表达也较缺血组及生理盐水组弱。结论:利多卡因预处理能显著改善脑缺血后神经功能缺损评分、减小脑梗死体积、减轻血脑屏障通透性及脑水肿,并与下调AQP4的表达有关。OBJECTIVE To observe the effects of lidocaine precondtitoning on the expressions of aquaporin4 during focal cerebral ischemia+reperfusion in rats,and to investigate the possible neuroprotective mechanism. METHODS Ninety-six male Sprague Dawley rats were randomly divided into four groups: sham operation group undergoing sham operation,ischemia reperfusion group undergoing thread embolism of the left middle cerebral artery occlusion(MCAO) to cause focal ischemia for 2 h and then undergoing reperfusion,lidocaine group and saline group undergoing intraperitoneal injection of lidocaine in doses of 10 mg.kg +^-1 or isovolumic saline 30 min before MCAO. At 72 h after reperfusion scores of neurological deficit were estimated, then the rats were decapitated, cerebral infarction volume, the contents of cerebral water and Evens blue(EB) were measured, the expressions of aquaporin4(AQP4) were examined using immuno-histochemistry and Western blotting. RESULTS The score of neurological deficit in rats and cerebral infarction volume, the contents of cerebral water and Evens blue in lidocaine group were lower significantly than those in ischemia and saline group,the expression of AQP4 was inferior than the above-mentioned two groups simultaneously. CONCLUSION Preconditioning with lidocaine can improve scores of neurological deficit post ischemia, diminish cerebral infarction volume,lessen blood brain barrier permeability or cerebral edema,, and this is related with the expressions of AQP4 which was down-regulated.
