单侧椎板切除减压治疗单侧腰椎管侧隐窝狭窄症的手术疗效分析

摘　　要：目的:比较单侧椎板切除和保守治疗对中度单侧腿疼的单侧腰椎管侧隐窝狭窄症患者的治疗效果,探讨单侧椎板切除在治疗单侧腰椎管侧隐窝狭窄症中的临床应用价值。方法:对28例中度单侧腿疼的单侧腰椎管侧隐窝狭窄症患者随机分为两组,一组采用单椎板切除减压手术治疗,另外一组行保守治疗,分别在术后6个月、12个月、24个月对患者的腰痛、下肢放射痛、行走耐受性和功能恢复进行评价。结果:术后各期随访单椎板切除减压手术治疗组有效率均高于保守治疗组(P<0.01),经统计学处理,两组之间有显著差异。结论:单侧椎板切除减压治疗单侧腰椎管侧隐窝狭窄症的减压效果是显著,并且术后的长期效果满意,并可避免因为全椎板切除而产生的并发症,可以作为治疗单侧腰椎管侧隐窝狭窄症的标准手术,在临床上推广。Objective：To compare the therapeutic efficacy between conservative therapy and unilateral laminectomy for unilateral lumbal lateral recess stenosis patients,and to explore the clinical value of unilateral lumbal lateral recess stenosis in patients with unilateral laminectomy.Methods：28 cases of unilateral lumbal lateral recess stenosis were divided into two groups,one group treated with unilateral laminectomy,and the other group treated with conservative therapy.The patients were evaluated by back pain,leg pain,walking tolerance and neurological during follow-up.Results：The unilateral laminectomy group produced a higher rate of improvement than the conservative therapy group during follow-up.There were significant difference between the two groups.Conclusions：Conclusion： Unilateral lubal lateral recess stenosis can be decompressed adequately by unilateral laminectomy,which obtained satisfactory long-term outcomes with few complications and low cost,can be a standard procedure for the surgical treatment of the unilateral lubal lateral recess stenosis.
