面部肥厚型鲜红斑痣的手术治疗

摘　　要：目的总结头面部肥厚型鲜红斑痣的治疗经验，探讨其手术方式的选择及疗效。方法自1990年1月至2010年12月，共收治126例头面部肥厚型鲜红斑痣患者，采用单一术式或联合多种术式进行个性化治疗。其中，行选择性切除者3l例；完整切除＋分区植皮者59例；选择性切除＋局部皮瓣＋分区植皮者13例；完整切除＋局部皮瓣十分区植皮者23例。结果本组126例患者中，术后切口感染者3例，切口裂开者1例，皮片部分坏死者9例，局部复发者14例，均经对症处理后患者满意。局部皮瓣完全成活。术后随访所有患者3个月至2年，效果均较满意。结论根据患者的病变特点选择相应的手术方式，可使面部肥厚型鲜红斑痣的治疗取得良好的效果。Objective To summarize experiences of surgical treatment about hypertrophic port wine stains and explore the selection of operative methods and its effects. Methods From January 1990 to December 2010, retrospective analysis was performed on 126 cases with hypertrophic port wine stains on the face. Of them, selective resection was used in 31 cases; complete resection and divisional skin grafting in 59 cases; selective resection and divisional skin grafting in 13 cases; selective resection, local flap and divisional skin graft- ing in 23 cases. Results Of 126 cases, wound infection occurred in 3 cases, disruption of wound occurred in 1 case, partial skin necrosis appeared in 9 cases and postoperative local recurrence happened in 14 cases, which were healed by non-surgical treatments. All the local flaps survived. All patients were satisfied with the outcome after 3 to 24 months following up. Conclusion According to the characteristics of the lesions, selection of reasonable operations for the treatment of hypertrophic port wine stain can obtain good results.
