加味麻杏石甘汤在喉部分切除患者术后的使用价值

摘　　要：目的:观察加味麻杏石甘汤剂对喉部分切除患者术后临床症状、体征和误咽程度的影响。方法:将60例喉部分切除患者随机分为治疗组和对照组各30例。对照组给予常规的西医综合治疗(抗生素和气道湿化等的应用),治疗组在一般西医治疗的基础上加用具有健脾、清肺、化痰作用的加味麻杏石甘汤剂(每天2剂),疗程7天,比较两组患者临床症状、体征的变化,并采用惯用的吞咽功能判定标准对术后患者误咽程度进行评估。结果:临床症状和体征(咳嗽、咯痰、喘息、气短、哮鸣音)方面,两组治疗后均有改善(与治疗前比较,P<0.01),且治疗组的改善作用优于对照组(P<0.05)。通过Ridit分析两组术后患者误咽程度,结果表明治疗组轻于对照组,两组之间差异有统计学意义(U=7.341,P<0.001)。结论:在常规西医治疗的基础上加用健脾、清肺、化痰中药治疗喉部分切除术后患者,较之单纯使用西药治疗能更好地改善患者的临床症状和体征,并减轻误咽程度,促进喉功能的恢复。联合运用加味麻杏石甘汤治疗喉部分切除术后患者的疗效确切,可以值得临床应用。Objective：To study the curative effect of modified Maxingshigan Decoction in postoperative patients of partial laryngectomy and to investigate variations of Clinical symptoms,signs and the extent of deglutition disorder after operation.Methods：60 cases were divided randomly into the treatment group（additional traditional Chinese medicine group） and the control group（the western medicine group）.the control group were given the antibiotics、Airway humidification etc.And the treatment group were additionally used the modified Maxinshigan Decoction.There were some indicators detected before and after treatment including Clinical symptoms,signs and the extent of deglutition disorder after operation.Results：After the treatment,Clinical symptoms and signs（cough,expectoration,wheezing,shortness of breath,wheezing）,the two groups were improved after treatment（compared with before treatment,P0.01）,and the effect of the treatment group than the control group improved（P0.05）.The evaluation of deglutition disorder was analyzed by Ridit analysis in both groups and the results showed that there was significant difference between the two groups（U = 7.341,P0.001）.The symptom of deglutition disorder in the treatment group was significant less than in the control group.Conclusion：Modified Maxingshigan Decoction was effective in postoperative patients of partial laryngectomy.
