寒湿型腰椎间盘突出症电针和推拿不同干预次序组合疗效比较研究

摘　　要：目的:通过观察电针和推拿不同干预次序组合治疗寒湿型腰椎间盘突出症的疗效,为临床治疗寒湿型腰椎间盘突出症筛选最佳的治疗方案。方法:本研究采用随机对照法进行临床观察,将符合纳入标准的寒湿型腰椎间盘突出症患者40例,随机分为先电针后推拿组和先推拿后电针组各20例,比较两种不同干预次序组合治疗后,中医病证诊断疗效标准疗效评定和日本骨科学修订的下腰痛评分标准(JOA评分)的变化情况。结果:两组治疗后较治疗前JOA评分均有显著改善(P<0.05);治疗后JOA评分改善指数比较,先电针后推拿组优于先推拿后电针组,差异显著(P<0.05)。临床疗效比较,先电针后推拿组:痊愈7例、显效7例、有效5例、无效1例;先推拿后电针组:痊愈3例、显效5例、有效9例、无效3例。先电针后推拿组疗效优于先推拿后电针组(P<0.05)。结论:两种疗法治疗寒湿型腰椎间盘突出症均有显著疗效,但先电针后推拿的治疗效果明显优于先推拿后电针组。Objective：Through the observe of the clinical effect of Acupuncture and Massage combined in different turn for lumbar intervertebral disc herniation of cold-damp syndrome,to select the best clinical treatment for lumbar intervertebral disc herniation of cold-damp syndrome.Methods：This study used randomized controlled clinical observation of law,would be consistent with the inclusion criteria of lumbar intervertebral disc herniation of cold-damp syndrome patients 40 cases were randomly divided into acupuncture-before-massage group 20 cases and 20 cases of acupuncture-after-massage group.Comparing the combination therapy in different turn to observe the chinese medicine diagnosis disease effect efficacy of the standard assessment and the japanese orthopaedic association scores for assessment of cervical myelopathy（JOA）.Results：The two groups can be significantly improved in the JOA scores after treatment（P0.05）;and the group of acupuncture-before-massage was more significantly improved in the JOA scores than the group of acupuncture-after-massage（P0.05）.In comparing of clinical efficacy,the group of acupuncture-before-massage was,7 cases of cured,7 cases of markedly effective,5 cases of valid and invalid one case;better than the group of acupuncture-after-massage that was 3 cases of cured,5 cases of markedly effective,9 cases of valid and invalid 3 cases（P0.05）.Conclusion：Two types of treatment for lumbar intervertebral disc herniation of cold-damp syndrome are both significant effective,while the group of acupuncture-before-massage is better than the acupuncture-after-massage one.
