胃大部切除术治疗胃十二指肠溃疡大出血的疗效分析

摘　　要：目的探讨采用胃大部切除术治疗胃十二指肠溃疡大出血的临床疗效,为治疗胃十二指肠溃疡大出血提供参考。方法分析2009年3月～2010年4月收治的51例采用胃大部切除手术治疗胃十二指肠溃疡大出血的患者资料,总结患者的治疗效果及术后恢复情况。结果 51例患者均得到了临床的有效救治,其中采用择期手术者5例,采用急诊手术46例,所有患者均得到治愈。患者手术时间平均为(52.33±8.18)min,术中平均出血量为(132.75±11.32)mL。患者胃肠功能在术后16～48h内恢复,胃肠减压平均持续时间为(2.65±0.33)d,患者住院时间平均为(8.68±1.59)d。结论采用胃大部切除术治疗胃十二指肠溃疡具有较好的疗效,患者痊愈率较高,手术安全可行,值得在临床推广使用。Objective To explore clinical efficacy of gastrectomy in the treatment of bleeding of gastric ulcer and provide the reference for the treatment of gastric ulcer bleeding.Methods The data of fifty-one patients with gastric ulcer bleeding in our hospital from March 2009 to April 2010 treated with subtotal gastrectomy surgery were analyzed,summarize the treatment effect of patients and postoperative recovery.Results Fifty-one patients had been clinically effective treated,elective surgery was used in 5 cases,the use of emergency surgery in 46 patients,all patients were cured.The average time for surgery was（52.33 ± 8.18） min,mean intraoperative blood loss was（132.75 ± 11.32） mL.Gastrointestinal function in patients 16 to 48 hours after recovery,the average duration of decompression was（2.65 ± 0.33） days,the average hospital stay of patients was（8.68 ± 1.59） days.Conclusion The use of subtotal gastrectomy for the stomach and duodenal ulcer treatment has better efficacy,a higher cure rate of patients,surgery safe and reliable,worthy of clinical use.
