关节镜下同种异体肌腱移植修复膝交叉韧带早期排斥反应的观察

摘　　要：目的了解关节镜下同种异体肌腱移植修复膝交叉韧带术后早期排斥反应的发生情况。方法对关节镜下行同种异体肌腱移植修复膝交叉韧带的17例患者(异体肌腱移植组)的临床资料进行分析,以同期29例自体肌腱移植患者(自体肌腱移植组)作为对照,比较两组术后早期体温、关节疼痛、肿胀积液及外周血白细胞计数的差异。结果异体肌腱移植组手术当日至术后14 d的最高体温与自体肌腱移植组接近。自体肌腱移植组术后平均发热4 d,异体肌腱移植组术后平均发热3 d;两组术后第4日体温比较差异有统计学意义(P<0.05);术后5 d以后最高体温均在37℃以下,没有出现二次体温升高(双峰)的表现。两组术后出现关节疼痛、关节腔积液和外周血白细胞升高的百分率比较差异无统计学意义(均为P>0.05)。结论关节镜下同种异体肌腱移植修复膝交叉韧带术后早期排斥反应不明显。Objective To investigate occurrence of early rejection after arthroscopic cruciate ligaments reconstruction with homologous tendon transplantation.Methods Clinical data of 17 cases undergoing homologous tendon transplantation for cruciate ligaments reconstruction（homologous tendon transplantation group）were analyzed.And 29 cases of autograft tendon transplantation（autograft tendon transplantation group）were choosed as control group.The body temperature,joint pain,arthroedema and white blood cell（WBC）count of peripheral blood were compared in two groups.Results The peak temperature in homologous tendon transplantation group was similar to that in autograft tendon transplantation group from operation day to day 14 after surgery.The average duration of fever was 4 days in autograft group and 3 days in homologous group.There was significant difference in body temperature on day 4 after surgery between the two groups（P〈0.05）.On day 5 post-operation,the body temperature of patients in both groups was below 37 ℃ without fervescence（double peak） again.There were no significant differences in morbidity of joint pain,arthroedema and WBC count rising between two groups（all in P〈0.05）.Conclusion Early rejection is mild after arthroscopic cruciate ligaments reconstruction with homologous tendon transplantation.
