尿MA、β_2-MG与血清Hs-CRP联合诊断2型糖尿病早期肾损害的临床价值

摘　　要：目的探讨2型糖尿病(T2DM)患者尿微量白蛋白(MA)、β2微球蛋白(β2-MG)和血清超敏C反应蛋白(Hs-CRP)与糖尿病早期肾损害的关系。方法测定116例T2DM患者(包括92例无早期肾损害T2DM患者和24例糖尿病肾病患者)及35例健康对照者的尿MA、β2-MG、血清Hs-CRP的水平,并进行统计学分析。结果 T2DM组尿MA、β2-MG、血清Hs-CRP检测结果均明显高于健康对照组(P<0.05);早期糖尿病肾病患者尿MA、β2-MG、血清Hs-CRP显著高于无肾损害的糖尿病患者(P<0.05),三项联合检测阳性率达70.65%,明显高于单项检测(P<0.05)。结论尿MA、β2-MG是反映早期糖尿病肾损害的敏感指标,对2型糖尿病患者采取尿MA、β2-MG与血清Hs-CRP联合检测,对糖尿病肾病早期肾损害的诊断监控及早期防治有较高的应用价值。Objective To explore the relationship between urinary microalbuminuria （ MA）, β2- microglobulin （ β2- MG）, serum high - sensitive C - reactive protein （ Hs - CRP） and diabetic nephropathy （DN） in type 2 diabetes mellitus （DM） patients. Methods The levels of urinary MA, β2- MG, and serum Hs - CRP were detected in 116 patients （92 DM, 24 DN） and 35 healthy subjects. Results Compared with normal controls, the levels of urinary MA, β2- MG, serum Hs - CRP in DM patients were significantly higher（ P 〈 0.05 ）. The levels of urinary MA, β2- MG, serum Hs - CRP in DM patients with DN were obviously higher than those in DM patients without DN（ P 〈0.05）. Moreover, the positive rate of combined detection reached 70.65%, which was significantly higher than that of the individual testing （ P 〈 0.05 ）. Conclusion The levels of urinary MA, β2-MG, and serum Hs - CRP are sensitive indicators of early diabetic nephropathy. And combined detection is valuable for diagnosing and monitoring early diabetic nephropathy.
