228例Ⅰ类切口手术抗菌药物预防应用的调查分析

摘　　要：目的调查医院疝气修补术、甲状腺及乳腺3种Ⅰ类切口手术围手术期抗菌药物的预防应用,并评价其合理性。方法回顾性调查2009年11月-2010年10月3种Ⅰ类切口手术住院患者228例,对患者的基本资料、疾病诊断、手术情况、用药时间、药物名称、用法用量等进行统计分析。结果Ⅰ类切口手术预防用药率达100.0%,使用抗菌药物频度居前3位的为二代头孢87例次、一代头孢42例次、三代头孢40例次;108例术前30min～2 h给予首剂药,占43.4%,35例首剂用药时间>术前24 h,术后平均用药时间3.5 d;单一用药占69.3%,二联用药占22.4%,≥三联用药占8.3%。结论Ⅰ类切口围手术期抗菌药物预防应用仍需加强管理,药物的选择及用药时间应遵守相关的指导原则。OBJECTIVE To investigate the present situation of per operative antibiotic prophylaxis in class I incision and evaluate it s rationality. METHODS A retrospective survey on the 3 types of classⅠ incision operations of 228 hospitalized patients from Nov 2009 to Oct 2010 in our hospital was carried out. The basic situation of the patient, disease diagnosis, surgical cases, medication time, medication name, dosage, etc were done by statistical analysis. RESULTS The rate of per operative antibiotic prophylaxis in class I incision was 100%, the frequency of antimicrobial agents usage was 2nd generation cephalosporins （87 cases）, 1st generation cephalosporin （42 cases）, third generation cephalosporin （40 cases）; the first dose of antibiotic started at 0.5-2h before surgery was 108 patients, accounting for 43.4%, while the first medication time of 35 cases was above 24 hours before surgery, after surgery the shortest antibiotic treatment was 1 d and the longest was 8 d, the average treatment time was 3.5d; single drug accounted for 69. 3%, 22. 4% two drugs together, more than triple and the triple drug accounted for 8.3%. CONCLUSION The management of antimicrobial prophylaxis in class I incision Per operative period still need to be strengthened, the choice of drugs and medication time should comply with the relevant guidelines.
