摘  要:艾司洛尔 (Esm)作为超短效 β1 受体阻滞剂已广泛应用于临床。本文观察该药在国人的耐受情况及电生理效应。本文选择阵发性室上速患者9例 ,分三组进行耐受性试验 ,并观察药物的血液动力学效应。另选择室上速患者15例 ,单次注射后9min内进行电生理检查。试验剂量下艾司洛尔使心率明显下降 ,血压及外周血管阻力呈下降趋势 ,对心输出量无不良影响。药物可终止双径折返型室上速。正常人对艾司洛尔耐受良好 ,药物不良反应发生率低。Esmolol, an ultrashort-acting β1 adrenergic receptor antagonist, is widely used in different clinical situations. The tolerance, hemodynamic and electrophysiological characteristics of intravenous esmolol in Chinese were investigated in this study. 24 patients with paroxysmal supraventricular tachycardia and normal ventricular function were included in this study. 9 of these patients, divided into three groups, received 500 μg·kg-1·min-1 esmolol as bolus for one minute followed by continuous intravenous infusion of 25 μg·kg-1·min-1, 100 μg·kg-1·min-1 or 200 μg·kg-1·min-1 for 20 minutes, respectively. The other 15 patients received bolus with esmolol 500 μg·kg-1·min-1 for one minute only. The electrophysiological studies were carried out in each patient within 3~9 minutes after the beginning of intravenous infusion. In patient with paroxysmal supraventricular tachycardia and normal cardiac function, intravenous esmolol decreased heart rate. There were trends to decrease mean blood pressure and peripheral vascular resistance. There was no harmful effect on cardiac index. The drug had no effect on pulmonary capillary wedge pressure. In electrophysiological study through blocking the anterograde conduction over the slow conduction pathway, single dose of esmolol could conversion the cardial rhythm to normal sinus rhythm in patients with AV node reentry tachycardia. In our study, we did not find the effect of esmolol on atrial-ventricular reentry tachycardia. With the doses approved in this study, intravenous esmolol could be well tolerated with no patient experiencing hypotension.
