摘 要:目的研究早产儿取不同体位及床角度对心率、呼吸、SpO2、呼吸暂停及胃潴留的影响。方法选择2009年8月-2010年8月我院新生儿科收治的90例早产儿为研究对象,随机分为0°仰卧位组、0°俯卧位组、0°侧卧位组及30°仰卧位组、30°俯卧位组、30°侧卧位组。各组在常规护理基础上,分别观察1 h、2 h、3 h、4 h、5 h、6 h其心率、呼吸、SpO2、呼吸暂停、胃潴留情况。结果 3-6 h 0°俯卧位组、30°仰卧位组、30°俯卧位组、30°侧卧位组患儿SpO2增加,未出现呼吸暂停,胃潴留量少(P〈0.05)。结论抬高床脚至30°取仰、俯、侧卧位,有益于早产儿生长发育,值得推广。Objective To evaluate the effects of different body position on the heart rate, respiratory rate, SpO2, apnoea and gastric retention in preterm infants. Method 90 preterm infants from Aug. 2009 to Aug. 2010 in neonates division were randomly divided into 0° supine group, 0° prone group, 0° tilt group; 30° supine group, 30°prone group, 30° tilt group. The heart rate,respiratory rate, SpO2 ,apnoea and gastric retention were recorded at 1 h, 2 h, 3 h, 4 h, 5 h, 6 h. Result The heart rate, respiratory rate and gastric retention in 0°prone group, 30° supine group, 30° prone group and 30° tilt group at 3-6 h were significantly lower than that of 0° supine group and 0° tilt group (P〈0.05) The SpO2, was significantly higher in 0° prone group, 30° supine group, 30° prone group and 30° tilt group than that in 0° supine group and 0° tilt group (P〈0.05). Conclusion There is a number of physiological benefits for infant growth development in 30° supine group, 30° tilt group and prone groups. It was worthy of promotion.
