摘 要:目的探讨血清透明质酸(HA)和Ⅲ型前胶原氨基端肽(PCⅢNP)预测早产儿支气管肺发育不良(BPD)的价值。方法选择2006年1月至2010年9月在我科住院、胎龄<32周或体重<1500g的早产儿,分别检测生后第1、14、28、42天血清HA和PCⅢNP水平,其中诊断BPD的早产儿为BPD组,从未合并BPD的早产儿中随机选取30例与BPD组胎龄、体重、性别相匹配的早产儿为对照组。比较两组HA和PCⅢNP的动态变化。结果 BPD组出生第1天血清HA和PCⅢNP水平与对照组差异无统计学意义[(86.3±5.2)μg/L比(90.6±6.6)μg/L,(8.1±1.3)μg/L比(7.6±2.1)μg/L,P均>0.05],生后第14、28、42天明显高于对照组[HA:(185.3±8.2)μg/L比(95.0±6.3)μg/L,(251.7±6.4)μg/L比(98.3±5.3)μg/L,(168.8±5.5)μg/L比(96.4±4.7)μg/L;PCⅢNP:(30.6±3.2)μg/L比(10.1±1.1)μg/L,(65.6±4.5)μg/L比(10.3±3.0)μg/L,(25.6±2.2)μg/L比(10.6±2.2)μg/L,P均<0.05];BPD组血清HA和PCⅢNP水平在28天内呈进行性上升,28天后逐渐下降。结论血清HA和PCⅢNP水平有助于早产儿BPD的早期预测。Objective Explores the significance of changes in Hyaluronic Acid(HA) and N-terminal pro-peptide of type Ⅲ collagen(PCⅢNP) in serum among preterm infant developing bronchopulmonary dysplasia(BPD),and their predictive value.Methods Nineteen preterm infants with BPD were selected for experimental groups and thirty without BPD were contro1 groups,and all birth weight were less than 1500 grams with a gestational age less than thirty-two weeks.Two 2ml serums were taken and measured the concentrations of HA and PCⅢNP on the 1st,14th,the 28th,the 42nd day after birth.Results The levels of HA and PCⅢNP were no significantly different between experimental group and control group on the first day after born [(86.3± 5.2)μg/L vs.(90.6± 6.6)μg/L,(8.1± 1.3)μg/L vs.(7.6± 2.1)μg/L,all P〈0.05].The levels of HA and PCⅢNP in preterm infant of BPD group were significantly higher than in control group on the fourteenth day,the twenty-eight day,the forty-second day after born[HA:(185.3± 8.2)μg/L vs.(95.0± 6.3)μg/L,(251.7± 6.4)μg/L vs.(98.3± 5.3)μg/L,(168.8± 5.5)μg/L vs.(96.4± 4.7)μg/L;PCⅢNP:(30.6± 3.2)μg/L vs.(10.1± 1.1)μg/L,(65.6± 4.5)μg/L vs.(10.3± 3.0)μg/L,(25.6± 2.2)μg/L vs.(10.6± 2.2)μg/L,all P〈0.05].The levels of HA and PCⅢNP in preterm infant of experimental group were progressive rising within twenty-eight days after born,then fall gradually.Conclusions The level of HA and PCⅢNP in serum has prognostic value in preterm infants bronchopulmonary dysplasia.
